Application of PRP and platelet-rich fibrin (PRF) in cosmetic gynecological indications

Objectives: Discuss the effect of regenerative medicine PRP/PRF in different Gynecological indications and conditions that affect the female reproductive system, including stress urinary incontinence, vaginal atrophy, and sexual dysfunction, as a stand alone or add on to the traditional treatment options for these conditions.

Introduction: Platelet-rich plasma (PRP) and platelet-rich fibrin (PRF) are autologous blood-derived products that have gained significant attention in various medical fields due to their regenerative and healing properties. While primarily used in orthopedics and dermatology, PRP and PRF have recently emerged as promising therapies in the field of gynecology.

Materials / method: PRP and PRF have shown promise in several areas. In the management of post surgical or post laser PRP have been investigated to enhance the efficacy of surgical repairs and promote tissue healing. For stress urinary incontinence, PRP has been explored as an adjunct therapy to augment surgical procedures or as a standalone treatment, aiming to strengthen the urethral and pelvic floor muscles. Additionally, PRP and PRF have been considered as potential options for addressing vaginal atrophy and sexual dysfunction by promoting vaginal tissue rejuvenation and improving local blood supply.

Results: Preliminary studies and anecdotal evidence suggest that PRP and PRF therapies in gynecology are generally safe and well-tolerated, with minimal adverse effects reported. However, more rigorous clinical trials and long-term follow-ups are needed to establish their efficacy, optimal dosing protocols, and standardized treatment regimens.

Conclusion: The application of PRP and PRF in gynecological indications holds promise as a non-invasive or adjunctive treatment option. These autologous blood-derived therapies have the potential to promote tissue regeneration, enhance healing, and improve symptoms in various gynecological conditions. Continued research and clinical trials will help further elucidate their effectiveness, ensuring evidence-based integration into gynecological practice.

Effect of DBD plasma on wound healing after labiaplasty

Objectives: The effect of DBD plasma therapy on wound healing after labiaplasty.

Introduction: Background: It is important for a surgeon to control complications such as redness, swelling, itching, discharge and especially dehiscence after labiaplasty. DBD (dielectric barrier discharge) plasma therapy, which can be a less complicated and safe method in controlling skin complications and accelerating wound healing, was used in the present study for patients underwent labiaplasty and its effect on the mentioned complications was investigated.

Materials / method: Methods: In this quasi experimental study, 40 patients aged 30-40 who had undergone trim labiaplasty surgery were recruited. None of them had medical history, smoking, or medication. DBD plasma therapy was performed on days 3, 7 and 10 after the surgery. Each session lasted for 10 minutes. The operation site was assessed for redness, secretion and itching on days 7 and 14, and the wound was reassessing for dehiscence until day 21.

Results: Results: Following the DBD plasma therapy that started on the 3rd day after the labiaplasty and was repeated on the 7th and 10th days, the wound assessment on the 7th day showed that 2.5% of the women's wounds had high level of secretions, 20% had moderate level of secretions, and 77% had low secretion, while on the 14th day, these values reached zero, 5% and 95%, respectively; and these changes were statistically significant (P<0.05). The redness of the wounds decreased from 5% among cases on the 7th day to zero on the 14th day (P<0.001).

Conclusion: Conclusion: Considering the results of this study, DBD plasma therapy can help the surgeon to control post-operative wound complications, provide women's health and subsequently improve their satisfaction with the changes made. Therefore, the use of DBD after surgery can make labiaplasty the satisfactory experience for both the surgeon and the patient.

Combined HIFU with systemic marine collagen vs. HIFU in vaginal rejuvenation with reflection on sexual satisfaction and urinary incontinence

Objectives: To investigate the efficacy of HIFU with oral intake of sachets containing super collagen with vitamins C and E vs. HIFU alone in vaginal tightening on sexual satisfaction and urinary incontinence.

Introduction: Vaginal rejuvenation is a current marketing term that incorporates not only aesthetic operations, such as tightening, but also procedures intended to address dyspareunia, urine incontinence, dysuria, vulvar irritation, orgasmic dysfunction, dryness, or atrophy.Since the 1960s, clinical applications of high-intensity focused ultrasound (HIFU) for the noninvasive treatment of diseases in many organs have been widespread. It can reach deeper layers of skin that radio frequency and laser cannot.

Materials / method: Patients with "loose vagina" and lower sexual satisfaction who stated a wish to enhance their vaginal tightness participated in this research. There were 54 patients involved. Doctors apply ultrasound gel to a cartridge before inserting it into the vagina and radiating the circumference with HIFU radiation. A group of patients was treated with HIFU alone (group I), and another group was treated with HIFU and systemic marine collagen (group II). We evaluated the patient's vaginal pressure, preoperative safety, and sexual function.

Results: There was no significant difference between both groups regarding vaginal pressure before treatment, but there was a significant difference after treatment. The percent of change was significantly higher in group II than group I.

Conclusion: HIFU with oral ingestion of collagen resulted in significantly greater improvements in vaginal pressure compared with HIFU alone, with improvements in sexual function and urinary incontinence.

Use of botulinum toxin in aesthetic medicine and gynecology: controversies, and future directions

Objectives: The objective of this study is to point out which aspects need to be more carefully studied in order to ensure a consistent use of BoNT in gynecology.

Introduction: Numerous studies have been conducted to explore how BoNT can be applied, and it is becoming popular for treating various disorders such as chronic pelvic pain, vestibulodynia, and vaginism. However, the exact dosage and ideal location for injections still need to be clarified.

Materials / method: This review looks at the use of botulinum toxin in the gynecological field with the aim of determining what needs to be further investigated to achieve a standardized application.

Results: On the one hand, the summary of the available pieces of evidence that we collected in this overview suggests that the use of Botulinum toxin in gynecology needs further investigations to clarify indications, feasibility, safety and cost-effectiveness compared to other available approaches. On the other hand, the robust experience gained in the field of aesthetic medicine may be partially translated in the gynecological field and pave the way for proper and evidence-based trials.

Conclusion: In the current scenario, we take the opportunity to solicit a close collaboration between experts in aesthetic medicine and gynecology, in order to adopt clear indications, reproducible clinical application, and implement the best quality management for our women.

Systematization of exosomes as drug delivery for technologies in genital area

Objectives: The objective is to establish a standardization of technologies that would best serve as drug delivery in each type of treatment for intimate rejuvenation, identifying the most suitable treatments for each condition.

Introduction: The use of exosomes in dermatology, particularly in the treatment of genital areas, represents a significant innovation. This technology has shown potential in promoting skin regeneration, rejuvenation and a drug delivery, as well as in addressing specific conditions, including vulvar bleaching, hydration, skin tightening, vulvovaginal atrophy and specific conditions as scleroatrophic lichen.

Materials / method: We divided the patients' concerns into the following topics:\

Vulvar Bleaching– microneedling or microneedling RF with exosomes combined with picosecond laser and peels
Vulvar laxity – microfocused ultrasound and/or radiofrequency plus electroporation exosomes.
Scleroatrophic lichen – CO2 laser and/or Erbium laser and/or radiofrequency with electroporation exosomes
Vulvovaginal Atrophy – CO2 or Erbium laser and exosomes
Improved Sexual Activity – radiofrequency with electroporation exosomes plus eletromagnetic field
Stress Urinary Incontinence – CO2 or Erbium laser plus eletromagnetic fiel

Results: The results are based on the authors' experience of over 10 years in this field and, in this case, on the use of exosomes in intimate rejuvenation for at least 10 months in conjunction with various technologies. The outcomes achieved surpassed those obtained when using the technologies individually and were rated as highly satisfactory by the patients when used with exosomes. No complications were observed in any of the treated cases with improvement outcome.

Conclusion: The use of exosomes is growing in the field of dermatology, however, there is still a need to standardize their use when combined with the technologies. Therapies, when used in combination as described in the text above, demonstrate superior and more satisfactory results, when used individually. No complications were recorded.

Prospective case series study presenting the outcome bipolar non-ablative RF in the treatment of vulvovaginal atrophy

Objectives: Cancer is not only a leading cause of mortality, but also causes countless negative consequences related to sexual, mental and social functioning in both women and their partners. Among women who survived cancer, treatment consequences and sexual dysfunctions are a serious problem affecting sexual well-being among women. As the percentage of women surviving after malignancies increases, quality of life has become an important clinical issue. This article to present non-hormonal treatment for valvovaginal atophy.

Introduction: Cancer and cancer treatment has countless negative consequences; loss of sexual function leads to negative impact on the quality of life (QOL) and ranks 5th highest unmet need among cancer survivors. This prospective case series analysis presents the outcome and the improvement of the sexual function consequently the QOL after RF therapy in uterine cancer treatment induced VVA, using the validated tool of female sexual functioning index (FSFI) and QOL score (QOLS) analysis at 6 weeks.

Materials / method: Over a period of 12 months from June 2021, 35 adenocarcinoma uterine cancer survivors data of FSFI and QOLS were collected and studied before and after intravaginal and valval radiofrequency (RF) treatment, all cancer cases were treated radically with a curative intend as per European Society of Medical Oncology (ESMO) Guidelines. RF was performed using bipolar non-ablative device with a
sterile purpose-made gynaecological applicator. Output frequency was set at 480 kHz and temperature during treatment did not exceed 40 Celsius degrees per session. The course delivered in 4 session.

Results: All 35 cases has shown mean improvement in the FSFI from 5.5 (4-7) points to 30 (28-34) points
reflecting to a better sexual function and an improvement in the QOL by 70% or more gaining 7 to 8 points in the QOLS was also observed at 6 weeks after treatment. There was no immediate or delayed adverse events observed to date.

Conclusion: From this prospective data presentation we can observe the use of fractionated bipolar non-ablative RF can provide a safe and non-hormonal remedy to cancer treatment induced VVA and sexual dysfunction in endometrial cancer survivors with a positive reflection on QOL. Randomised control trials would be recommended in this field.
