Quality is underestimated - Improving skin behavior by the delivery of active substances

Objectives: 1. Mesotherapy – the importance of active substances (vitamins, amino acids, and growth factors) in skin physiology and skin aging.
2. Delivering active substances by topical application, injection, or with the assistance of microneedling and energy-based devices (focusing on mechanical thermal action)
3. Implication in skin rejuvenation (stand-alone/adjuvant), decreasing post-procedural downtime, and treating skin pathologies (stretch marks and hyperpigmentation)

Introduction: Most of the efforts in aesthetic medicine are directed toward quantity improvement, such as increasing collagen fibers, decreasing amount of pigment, and increasing volume in lost spaces.
Skin beauty is also depended on its quality and functionality. For example, high collagen can be also found in scars.
Some pathologies of the skin, like stretch marks and hyperpigmentation, require changing skin behavior and quality rather than quantity, and some aesthetic interventions can be potentiated if skin cells will be supplied with their nutrients.

Materials / method: A literature review was conducted to compare different active substances that are important for skin function, and their clinical implications.
Afterward a literature review was conducted to evaluate different delivery modes of active substances (mesotherapy).
This knowledge was combined with my practical experience and my protocols that involve the combination of mesotherapy with other procedures and as stand-alone treatments.

Results: Vitamins, amino acids, minerals, and growth factors (self-produces as PRP, or synthetic) are important factors in many reactions in our skin.
1. Collagen synthesis require the continuous supplement of vitamins and antioxidant for its correct and efficient laydown.
2. Skin hyperpigmentation is partly created by the lack of antioxidation materials in the tissue.
3. Stretch marks and scars are low quality skin conditions that will benefit from the improvement of their quality.
4. Fat can become thinner only by supplementation of substance that increases its metabolism.

Conclusion: • In order to increase the efficacy of our treatments we must supply the skin with the correct nutrients that are needed for performing its functions.
• Different methods exist for delivering of active substances into the skin and different consideration should be taken into account in each of them.
• Post-procedural downtime might be shorter if the skin is supplied with the correct ingredients for faster healing.

Peeling's dos and don'ts

Objectives: Peeling is a method that is very inexpensive and has many therapeutic benefits, but unfortunately, many doctors and people are afraid of this method, and the most important reason for this fear is the lack of proper training.
In this lecture, I intend to present the important and key points of peeling to the participants so that they can benefit from the benefits of this method and stay away from its side effects.

Introduction: I have done the peeling method for about 5000 cases for more than 10 years
i have used this method for the treatment of scars;PIH; hyper pigmentation;melasma in these years.
I am fully aware of the benefits of peeling and I know its possible side effects, and I also experienced the method of managing side effects well.
We have the duty to transfer our own experiences to other doctors in order to spread science properly so that both society and doctors can benefit from it.

Materials / method: In this speech, I am going to present the following points about peeling by summarizing my experiences and previous articles:
What is peeling;What is the reason for fear of peeling
Peeling application;Peeling case selection;Peeling materials
Peeling management;Combination therapy with peeling
peeling case management
All the mentioned points will be completely practical for doctors and they are presented in such a way that they can use this method correctly as soon as possible

Results: I have done this training method in Iran for about 300 doctors and I have achieved very good results
In this group of doctors, the fear of peeling decreased by about 75%, and performing this procedure in this group increased by about 43%, and the amount of complications from peeling in this group decreased by about 37%.
Participating doctors reported to me that the level of client satisfaction with peeling treatment has increased by about 68% compared to previous treatment methods.

Conclusion: Peeling is a very useful treatment method that we have a duty to use correctly in order to treat our clients
It is our duty to take care of the health and beauty of our clients, and the first step in this direction is proper education.
This lecture has been collected using the experiences of about 5000 peeling cases as well as previous articles to provide correct and comprehensive training.

Association of the perioral phenol-croton peel technique with HA filler in lip rejuvenation

Objectives: This case report aims to demonstrate a new technique for lip rejuvenation,
based on the association of perioral phenol-croton peel with lip filler with hyaluronic acid, which
allows treating all components of lip aging And providing highly satisfactory results for the patient.

Introduction: The aging of the lips leads to loss of volume, reduction in contour and formation of fixed rhytids in the perioral region. Treating this area is an important challenge due to the different components associated with its aging process. In this sense, phenol-croton peel allows effective treatment of deep rhytids in the perioral region, while hyaluronic acid fillers can be used to restore the contour, lost volume and restructure the lips.

Materials / method: The perioral area was marked using a white skin pencil. A 1% Hetter formula was applied thoroughly with a first pass using a wooden applicator with the wet tip wrapped in cotton, followed by 5-10 multi-direction passes without adding any extra solution. No analgesia is needed, as phenol itself works as an anesthetic
drug. After the healing process the lips was filled with hyaluronic acid using a 22 gauge cannula (image 1)

Results: This case report aims to demonstrate a new technique for lip rejuvenation, with long lasting results and providing highly satisfactory results for the patient.

Conclusion: The association of perioral phenol-croton peel with lip filler with hyaluronic acid is a new technique for lip rejuvenation, which allows treating all components of lip aging .

Outcomes of 97 patients who underwent phenol-croton peel: satisfaction index, short and long term side effects

Objectives: The authors aimed to analyze the satisfaction index of 97 patients submitted to the Phenol-croton peel procedure, in order to ascertain short- or long-term side effects, and satisfaction following the procedure.

Introduction: Phenol-croton peels are indicated for skin rejuvenation, a highly desired effect in the dermatological field. This procedure improves the appearance and texture of the skin by stimulating cell regeneration and reshaping. Phenol-croton peel is expected to promote excellent results in a relatively short time, with the potential to improve not only the appearance and lagginess, but also the patient's quality of life. However, studies published in recent years use relatively small samples, ranging from 9 to 16 patients.

Materials / method: We retrospectively reviewed 97 patients, with phototypes I-III, who underwent phenol-croton peel from 2015-2022. The study was approved by the Ethics Committee of Hospital Mário Pena and Plataforma Brasil. Data collection involved 97 participants who gave their written consent to the study and responded to the interview. Their responses were organized into groups according to the area where the peeling was performed (full face versus localized peel) and years of evolution after the procedure. The data were statistically analyzed, considering satisfaction and adverse events.

Results: On a scale of 1 to 5, 91.8% (n=89) of participants rated the procedure with a score higher than 4, with 69% (n=67) of them giving the highest score. Regarding adverse effects, only 12.4% (n=12) of the sample members reported these occurrences. In addition, there was a tendency towards a reduction of approximately 30% (5/17) in side effects after three months (p = 0.062), considering all treatments.

Conclusion: The majority of patients interviewed were satisfied with the procedure. Phenol croton peel, despite prolonged recovery time, leads to a high degree of satisfaction when well indicated among patients with phototype 1-3, with a 17,5% incidence of recent and 12,4% of late complications.
