Filler induced collagenesis and role in skin metabolism control

Objectives: This presentation aims at explaining the complex mechanisms involved in Filler induced Collagenesis. Knowledge of these elements allows for a better understanding of the Mechanotranduction factors and of some of the adverse events encountered.

Materials / method: Knowledge of the anatomical and physiological changes induced by tension mechanisms within the skin allows us to understand the modifications in the "youth appearance" of the skin through fillers injections

Results: The concept of "tension integrity" or "tensegrity" is based on the knowledge of "mechanotransduction" mechanisms, through which mechanical forces are changed into chemical signals to modify and regulate the cells, mainly the fibroblast.
These mechanisms are well studied in wound healing, and are the same that applied to the consequences of the Foreign Body Reactions, occurring when fillers are injected within the tissues.

Conclusion: It is therefore possible to influence the skin metabolisms in order to recreate a softer, more elastic, younger skin.
But the type of the collagen produced has a major influence on the quality of the final results, and should be taken into account to avoid some or treat some complications.

Challenging cases from IMCAS Alert

Objectives: 3 challenging cases from IMCAS Alert are presented for consideration.

Introduction: The reliability of any particular filler integrating is dependent on several interrelated factors. For optimal integration, the intrinsic biochemical property of the filler utilised should ideally be matched to the recipient tissue histo/biological bed (and indeed the anatomical tissue plane) into which it is injected. For example, high viscosity fillers injected into the subcutaneous malar tissue plane can result in sub-optimal outcomes including complications such as chronic oedema and Tyndall effect.

Materials / method: 3 Case Studies

Results: Many ancillary factors need to be considered including sterile technique; instrumental mode of injection (i.e., needle or cannula); atraumatic delivery; the localised vascular challenge; patient age; patient co-morbidities and of course ambient communicable disease and immunisations (such as the current pandemic). The aesthetic objective should be well planned to minimise disappointment. For example, filler over use may result in a poorly integrated outcome. It may be better to aspire to an objective outcome in serial filling episodes rather than trying to achieve everything desired at once.

Conclusion: Many clinical complications and challenges are submitted by colleagues to IMCAS Alert facility.
Collegiate responses generate intense discussion and invaluable assistance to colleagues in need of assistance. It is anticipated that the 3 cases selected for this webinar will create interest for all participants.
