Objectives: To understand the safety and efficacy of photodynamic therapy in the treatment of acne vulgaris
Introduction: Photodynamic therapy (PDT) in acne is widely recognised as a safe and effective method for treating both inflammatory and non-inflammatory lesions and different severity of acne from mild to severe.
In my presentation, beside a brief review of the literature on the application of PDT in the treatment of acne, I will show the promising results of an assessor blinded proof-of-concept trial showing the safety and efficacy of a novel formulation of 5-aminolevulinic acid 5% thermosetting gel in the treatment of acne.
Materials / method: 35 patients (13 male, 22 female), were treated using PDT with 5-aminolevulinic acid 5% thermosetting gel for a mean of 3 PDT sessions targeted to the face and held every other week (range 2-6 sessions). Global Acne Grading System (GAGS) score was recorded at the baseline and after each PDT session and at a 6 months follow up. A skin tolerability score (0-3 range) was reported by each patient after each PDT session.
Results: From a mean baseline GAGS score of 21 (SD +/-6), the GAGS score decreased to a mean value of 6.5 (SD +/- 5.7) after the last PDT session and mantained itself at a mean value of 6.7 (SD +/- 6.8) at a 6 months follow-up. Tolerability assessed by the patients was excellent, with all patients completing the study and with a reported discomfort mean level score of 0.5 (SD +/-1) on a scale from 0 to 3 (where 0 is no discomfort at all and 3 is the worst possible discomfort). No adverse events were reported.
Conclusion: PDT using 5-aminolevulinic 5% thermosetting gel is safe and effective for the tratment of mild to moderate acne vulgaris. The small size of the study is a limting factor. The fact that GAGS calculation included the chest and back, that were not treated during the study (only the face was treated), raises the possibility that the results could have been even better including chest and back in the treatment protocol.
