Alopecia: protocols of correction with HA and sodium succinate

Objectives: To identify improvements of hair growth after the procedures performed with the use of the preparation, containing hyaluronic acid and sodium succinate

Introduction: After skin diseases, such as seborrheic dermatitis, where we have diffuse alopecia, and alopecia areata, in the skin we observe changes in the microcirculatory, in some cases dehydrated skin, metabolic changes, chronic inflammatory. In both this cases patients completes of hair loss.

Materials / method: We observed 15 women aged 30 to 45 years with diffuse and alopecia areata. All women received 3 procedures, using a combined preparation containing sodium succinate (16 mg / ml) and hyaluronic acid (11 mg/ml or 18 mg / ml). The drug was administered intradermally in the technique of papular injections with a 30G needle, 2.0 ml per procedure, at intervals of 1 every 2 weeks.

Results: All patients showed a positive dynamic: hair growth, hair quality improvement. In cases of seborrheic dermatitis moisturizing effect was noted. Allergic reactions to drug administration have not been recorded. Absence of effect and negative dynamics during therapy were not revealed.

Conclusion: The protocol of using hyaluronic acid and sodium succinate has a good tolerance, promotes the hair growth and improve the condition of the skin.

Autologous cellular method using micrografts of human adipose tissue-derived follicle stem cells in androgenic alopecia

Objectives: The authors reported the microscopic and trichoscopic results of an autologous cell biological technique to compare, through histological,
immunocytochemistry, and cytospin analysis, hair re-growth obtained by micro-grafts from scalp tissue containing Human Intra- and Extra-Dermal Adipose Tissue-Derived Hair Follicle Stem Cells (HD-AFSCs) versus placebo (saline solution).

Introduction: Hair bio-engineering has risen at the crossing point of various manipulations to meet a clinical requirement for innovations to advance hair growth.

Materials / method: An autologous solution of micro-grafts was obtained from mechanical fragmentation and centrifugation of scalp biopsy’s (2 2 mm) using “Gentile protocol”. The micro-grafts solution was mechanically infiltrated on half of the selected patients’ scalps with Androgenic Alopecia (Norwood–Hamilton 2–5 and Ludwig 1–2). The other half was infiltrated with saline solution. Three injections were performed to each patient at 45-day intervals.
Of the 35 patients who were enrolled, 1 was excluded and 1 was rejected.

Results: 23 and 44 weeks after the last micro graft’s injections, the patients displayed a hair density improvement, with a mean increment of 33% 7.5% and 27% 3.5% respectively, contrasted with baseline values, for the treated region. Microscopic assessment appeared, in scalp biopsies, to show an expansion in the number of hair follicles per mm2 following 11 months from the last micro-grafts application compared with baseline (1.4 + 0.27 versus 0.46 + 0.15, respectively; p < 0.05).

Conclusion: HD-AFSCs contained in micro-grafts may represent a safe and eective alternative therapy option against hair loss.

Hair-aging and a specific anti-aging medical therapy

Objectives: The aesthetic and wellness medicine offer treatments to allow patients to feel good about their appearance and in harmony with themselves. Since the hair represents the frame of the face, modern anti-aging medicine approach should be focused both on skin and hair health and beauty, remembering that hair as part of skin appendages undergo inevitable aesthetic changes too.

Introduction: Like the skin, hair age for extrinsic damages (ie. UV rays and pollution), slowing of cellular turnover and hormonal changes. In women, first hair changes may occur from the age of 35, and worsen from around 40-45 years due to hormonal changes. The hair becomes ever thinner, dull and, in a widespread or localized manner, less dense. Seasonal fall or states of stress can cause hair loss which could lead patient to great distress and/or depression condition.

Materials / method: We evaluate and then proposed to our patients an injectable treatment specifically formulated and registered as medical device Class III for hair and scalp problems. This injectable in form of a solution contained in a bottle of 5 ml, has been developed in France and it is composed by natural HA and the registered RHB™ complex (6 Vit. B group, 5 amino acids and zinc).
The treatment consists of the infiltration of the solution “point by point” superficially over
the area to be treated using 30 or 32 G thin needles. The start protocol foresees 6 sessions (one every 7-10 days).

Results: The Speaker will underline the main advantages of this therapy (i.e. absence or minimal discomfort of the procedure, the quick response time especially related to hair loss decrease and to qualitative hair improvement), will show several results and explain the protocols adopted. In conclusion, he will underline how his experience is similar and confirm the results already registered in two international multicenter studies.

Conclusion: A versatile product that allows you to expand the aesthetic doctor's proposal because we do not forget that the importance of hair should not be overlooked in an anti-aging project.

Magic of growing hair - Use of peptides with microneedling of scalp in cases of androgenetic and female pattern alopecia

Objectives: 1. Treat cases of androgenetic and female pattern alopecia with microneedling of scalp.
2. Evaluate effect of peptide solution when used in combination with micronedling.
3.Use head massage to accentuate effect of peptides with microneedling.

Introduction: Hair being prime cosmetic concern for all,alopecia has been extensively studied and treated for past many years. One of the recent treatment options being microneedling of scalp. Microneedling acts by stimulating stem cells in dermal papilla by releasing growth factors. The micropunctures created by microneedling can be used to enhance delivery of nutrients to the hair follicle increasing the efficacy of the procedure. Peptides reduce the effects iof DHT on hair follicle and induce neovascularization. Head massage reduces the levels of stress hormone which is also responsible for hair loss.

Materials / method: 20 patients with various grades of androgenetic alopecia who were treated with minoxidil and oral vitamin supplements were included in this study .They were divided in two groups. Microneedling followed by head massage with hair cocktails containing peptides was done for 20 min for one group at a weekly interval for 6 weeks .The other group was treated with microneedling alone at the same interval. Minoxidil and oral vitamins were continued between and after the sessions.

Results: Results were compared at the end of 3 months and 6 months. The patients were evaluated based on hair count , standardized 7point visual analogue scale(-3 = greatly decreased ,-2 = moderately decreased, -1 = slightly decreased, 0 = no change, +1 = slightly increased, +2 = moderately increased, +3 = greatly increased) and patients’ subjective improvement.The group treated with peptide solution head massage along with microneedling showed statistically significant improvement as compared to the group treated with microneedling alone.

Conclusion: This study shows that head massage with hair cocktails containing peptides when done in the same session with microneedling accentuated the effect of microneedling on hair regrowth in cases of androgenetic alopecia and female pattern hair loss as compared to microneedling alone. Thus the triad of head massage, peptide solutions and microneedling is the best way to wish our patients HAPPY NEW HAIR.

Treating polycystic ovarian disease patients suffering from hairfall

Objectives: To get satisfactory improvement in hair restoration amongst Polycystic ovarian disease patients

Introduction: Polycystic ovarian syndrome ( PCOS) is known to be notorious when it comes to hairfall. Many causes and theories have been proposed in patients with polycystic ovarian disease, unfortunately none of the hypothesis clearly indicates the factors which lead to hairfall in certain patients. The worst part being, very few treatment options have been suggested to give results. We propose an algorithm to treat patients suffering from hairfall due to polycystic ovarian disease.

Materials / method: 100 patients with hairfall who were suffering from PCOS were evaluated after Ultra Sonography on 5th day of menstrual cycle. All these patients underwent hormonal investigations like follicular stimulating & luteinising hormone, total & free testosterone, dihydrotestosterone & prolactin, fasting insulin & others to rule out metabolic causes. For treatment, all these patients were put on Platelet Rich Plasma PRP therapy & flutamide 250 mg OD doses, topical minoxidil application & oral supplements biotin. Patients were assesed subjectively by blinded investigators & objectively by trichoscopy

Results: 88 % of patients showed excellent improvement in objective grading, 8% of patients showed good improvement, 2 % showed good improvement & 2 % patient showed no improvement in hair growth. In subjective assessment, 80% of patients showed excellent, 8% good improvement, 6 %average improvements & 6 %no improvements in hair growth. No major side effects related to PRP or flutamide or minoxidil was encountered during our study. Few patients complained of developing facial hair due to minoxidil application & 2 patients complained of putting on weight despite strict diest & exercise regime.

Conclusion: Combining Platelet Rich Plasma with a mild anti androgen like flutamide with topical minoxidil can do wonders in hair growth of patients suffering from Polycystic ovarian syndrome. In these patients this regime could be a new hope in hair growth as most of the stand alone modalities in PCOS patients fail to provide results.

Relationship between androgenetic alopecia and cardiovascular comorbidity markers in women

Objectives: Although androgenetic alopecia (AGA)is a cosmetic problem for lots of men and women, it’s also investigated about relations with systemic diseases. Severity of AGA was found related to coronary artery diseases in some groups

Introduction: AGA also shows a predisposition to cardiovascular heart diseases. These kind of studies are done usually among male patients with AGA. Female statististic data is limited about AGA and cardiac ischemic disease markers.Carotid artery intima media thickness was found directly related to symtomatic coronary artery diseases. In this study we investigated the correlations of some blood biochemical markers’ related to cardiac ischemic diseases and carotid intima media thickness values in females with AGA.

Materials / method: This study was done İn İzmir Tepecik Education and Investigation Hospital. Female volunteers with AGA older 18 years old were enrolled to the study.27 women were investigated. Mean of age was 40,93(22-66). Androgenetic alopecia were scored by Ludwig classification. Blood biochemical markers which are related to cardiac diseases were studied. These were serum total cholesterol, high-density lipoprotein (HDL);low-density lipoprotein ( LDL), Triglycerides,Dehydroepiandrosterone sulphate (DHEAS),total testosterone,CRP levels. As imaging carotid ultrasonography was used. Right and left commo

Results: In many volunteers elevated CIMT were observed. But we couldn’t find a linear correlation (p>0.05). The values of biochemical markers were not found related to severity of AGA and CIMT (p>0.05)

Conclusion: Patients of AGA must be also concerned about cardiac diseases. Measurement of CIMT in people with androgenetic alopecia (AGA) measurement can be helpful in determining risk of premature cardiovascular heart diseases. More studies are needed in this field.

Platelet-Rich Plasma (PRP) and its role in androgenetic alopecia treatment

Objectives: Objectives of the lecture are to present the PRP preparation and application technique and to assess the clinical efficacy and safety of PRP injections in patients with androgenetic alopecia. We will also discuss the advantages and disadvantages of PRP as monotherapy or as adjuvant treatment and we will compare it to other available androgenetic alopecia treatments.

Introduction: Androgenetic alopecia is the most common hair loss disorder affecting both men and women. Its treatment still remains a challenge for the dermatologists as available therapeutic options do not offer a satisfactory solution to the problem and adverse effects can be significant. PRP has attracted a lot of attention as a possible treatment for hair loss during the last decade. Nevertheless, there are a lot of controversies regarding preparation, application and treatment protocols.

Conclusion: PRP injections appear to be a potent safe and effective option for androgenetic alopecia, both in males and females. It appears to be an innovative and promising treatment in the dermatologists' armamentarium either as mono therapy or as adjuvant treatment.
