Objectives: 1. Identify and summarize the existing knowledge and research on the uses of supplements and diets modification in acne
2. Evaluate the efficacy of supplements and diet changes based on the evidence presented in the literature.

Introduction: Individuals are increasingly inclined to incorporate diet changes and supplements as potential treatment tools for their skin conditions. Seeking natural and holistic approaches, people are exploring dietary adjustments and supplement usage as complementary strategies to traditional skincare and medical interventions.Thus, it is crucial for dermatologists to understand the latest research and be able to engage in evidence-based discussions, offering reliable recommendations for supplements and diet changes. This knowledge empowers dermatologists to effectively guide their patients.

Materials / method: Literature review of the last 15 years was made using the terms "diet and dermatology" and "nutrition and skin", "nutritional agents and acne", "diet and acne", "supplements and dermatology", "supplements and acne", "omega 3 and skin", "probiotics and skin"

Results: There are numerous publications on the use of nutritional supplementation for dermatologic conditions. However, most are in vitro studies or association studies; there are very few high quality double blinded randomized controlled trials. There is some evidence for the use of supplements in acne. There is evidence linking decreased glycemic load and dairy avoidance with improved acne.

Conclusion: As the popularity of supplements continues to rise, dermatologists must understand potential benefits and limitations of these products. However, it is equally important to recognize the significance of diet changes as treatment options. Staying updated on the latest research enables dermatologists to offer accurate guidance to patients who may be considering supplement use or dietary modifications to enhance their skin health. By understanding both aspects, dermatologists can provide comprehensive and informed care tailored to the individual needs of their patients.

Objectives: Evaluate use of melon derived superoxide dismutase (SOD) oral nutraceutical combined with recognized treatments for vitiligo, melasma, and acquired skin pigmentation.

Introduction: Skin pigmentation disorders are difficult to manage, and standard treatments have limited efficacy. Vitiligo and Melasma are especially challenging disorders to treat, as well as non-genetic skin pigmentation issues. This presentation will review the use of a melon derived SOD oral nutraceutical for genetic and acquired pigmentary disorders.

Materials / method: Vitiligo: Prospective study (n=50) compared NB-UVB for 24 weeks or combined with SOD nutraceutical (500mg/day) and evaluated with Vitiligo Extent Scores. Melasma: Prospective study (n=47) compared triple cream (hydroquinone 2%, Tretinoin 0.05%, Fluocinoloneacetonide 0.01%) alone for 3 months, or combined with Beta-carotene (25,000 or 50,000 IU/day), or SOD nutraceutical (250mg or 500mg/day), evaluated with Melasma Area Severity Index. Acquired Pigmentation: Prospective study (n=23) compared placebo and SOD nutraceutical (500mg/day) for 90 days, evaluated by cross polarized standard Imaging usi

Results: Vitiligo: Prospective study (n=50) compared NB-UVB for 24 weeks or with SOD nutraceutical (500mg/day), evaluated with Vitiligo Extent Scores. Conclusion: This melon derived SOD oral nutraceutical makes NB-UVB treatment significantly more effective at improving re-pigmentation in vitiligo. It also significantly improves the effectiveness of topical therapy for melasma and is better than combination with standard dietary anti-oxidants. Additionally, the SOD nutraceutical alone significantly reduces pigmentation in non-genetic skin hyperpigmentation. Oral use of this SOD nutraceutical provides a powerful tool to treat both genetic and non-genetic pigmentation disorders.

Introduction: Melasma is always very difficult to treat and require safe and effective treatment.

Materials / method: The material is a new formulation of cysteamine in a specific vector system.
The clinical studies were done versus HQ as control and as reference.

Results: We got very good results with our any side effects.

Conclusion: For this indication of melasma we got no patient drop out in the studies and an excellent tolerance.

Objectives: 1.Understanding the core principles of a clinically effective exosomes cosmeceutical.
2. Understanding the concept of "loading of exosomes" with key dermatological actives to enhance the potency of exosomes for skin rejuvenation, skin brightening effect and reconstruction of the skin barrier.
3. Review of a number of cases using rose derived stem cells exosomes as a cosmeceutical.

Introduction: There are several challenges in the manufacturing of exosomes cosmeceuticals. Exosomes are nanosized vesicles that have clinical benefit only when a number of factors are respected; namely : they have to be derived from a stem cell culture, they have to be manufactured in a GMP manufacturing facility, they have to be refrigerated and they have to be lyophilised and they have to be appropriately loaded with actives. All these are challenges that have to be considered so that an exosomes cosmeceutical is clinically effective.

Materials / method: 2.5 billion rose stem cells exosomes were loaded with five synthetic growth factors, 6 peptides, nicotinamide, ascorbic acid, glutathione, vitamins and minerals.These rose stem cells exosomes were freezed dried to preserve the stability and the efficacy of the exosomes. A balm was designed containing tranexamic acid and niacinamide for skin brightening.The concept was reconstitute the exosomes with the balm at the time of application and to subsequently keep the product refrigerated to maintain their efficacy.The reconstituted cosmecutical is used within 10 to 14 day for maximum potency.

Results: A series of 20 patients have used 50mls of the reconstituted rose stem cells exosomes cosmeceutical patients over a period of 10 days twice daily. These patients had atopic dermatitis, psoriasis, rosacea, post procedure superficial burns after microneedling radiofrequency / laser resurfacing and skin sensitivity post chemotherapy.There was significant symptom relief with reconstruction of the skin barrier, anti-inflammatory effect, reduction in skin sensitivity and healing. Globally there was significant improvement in each of the cases with improvement in quality of life.

Conclusion: The core principles of exosomes manufacturing have to be respected so that a clinically effective exosomes cosmeceutical is produced. Loading of exosomes with actives will enhance the end results of the cosmeceutical. A clinically effective exosomes cosmeceuticals with refreshly reconstituted exosomes which is refrigerated would prove to provide symptomatic relief of several dermatological skin conditions and could also be used as a post procedure care after minimally invasive aesthetic procedures.

More research is required to potentiate which active can be successfully loaded into exosomes.

Objectives: There are still no clinical data on the anti-aging effect of oral treatment with retinol or retinal derivatives. For this reason, a prospective, randomized, controlled, 3-month, assessor-blinded, trial was conducted to assess the clinical efficacy and the tolerability of the In & Out strategy conducted with the combination of oral Vitamin A (50000 UI) and vitamin E (50 mg) and topical gel contains retinoic acid (0.02%) and glycolic acid (4%).

Introduction: Skin aging is caused by both intrinsic (chrono-aging) and extrinsic factors (photo-aging). Topical retinoid treatment is a standard therapeutic approach for skin aging. Vitamin A is a non-esterified free form of retinoid; retinol is able to stimulate new collagen deposition, reducing mettalloproteinases expression. Vitamin E has antioxidant properties. It is able to counteract radicals and protect the skin from solar radiation. Indeed, the use of antioxidants is an effective strategy to counteract the skin aging processes. Both vitamins A and E must be regularly consumed in our diet.

Materials / method: 60 participants, were enrolled and randomized in 1:1 allocation ratio: GROUP A applied retinoic acid (0.02%) and glycolic acid (4%) once a day every two days, GROUP B applied the medical device and assumed vitamin A (50000 UI) and vitamin E (50 mg) one capsule daily. Inclusion criteria were age>50 years, Glogau Score>2. Exclusion criteria were pregnancy/breastfeeding, liver disease, dyslipidemia, and acute skin diseases. Efficacy outcome was evaluated in an assessor-blinded at at T0, T1 (w6), T2 (w12)

Results: 60 participants were enrolled and completed the study and randomized into two groups: Group A (n=30, mean age 59.9±8.5 years, mean SAGS at baseline 12.8±4.0) and Group B. The Skin Aging Global Score (SAGS), based on the sum of different six parameters decreased in both groups: The reduction was statistically significant after 12 weeks in Group A (22%, p<0.05) and in Group B (-27%, p<0.01). The reduction in SAGS after 12 weeks compared to baseline was significantly higher in Group B. Both treatment regimens were well tolerated.

Conclusion: There are still no clinical data on the anti-aging effect of oral treatment with retinol or retinal derivatives. A prospective, randomized, controlled, assessor-blinded, trial was conducted to assess the clinical efficacy and the tolerability of the In & Out strategy conducted with the combination of oral Vitamin A and vitamin E and topical gel contains retinoic acid and glycolic acid. This trial confirmed that the combination of an “In & Out” strategy of oral supplementation and gel shows superior efficacy in terms of clinical and instrumental improvement in comparison with topical treatment.

Objectives: Lymphatic vessels are essential for skin health but their function deteriorates with aging and stress. We screened a library of 2200 natural plant extracts for the ability to promote lymphatic activity, using a 3D phenotype-based assay with human lymphatic endothelial cells. We identified 5 hits that were further characterized for their efficacy and underlying mechanisms involved in promoting lymphatic function in vitro. The feasibility to deliver tangible results using a combination of the lymphatic activators was confirmed in vivo, revealing efficacy against periorbital edema/puffy eyes.

Introduction: The lymphatic system governs fluid homeostasis, immune surveillance and the elimination of the skin’s metabolites and inflammatory mediators, thus playing a critical role in skin health and appearance. UV-exposure, stress and aging result in reduced function and rarefication of the lymphatic network with compromised drainage efficiency, thereby leading to impaired skin function and repair, wrinkles, sagging skin and puffiness. Given the importance of an efficient lymphatic system for healthy skin, we dedicated our work to the discovery of natural lymphatic activators.

Materials / method: Cryopreserved human lymphatic endothelial cells (LECs) were used, isolated as previously described (Hirakawa et al., 2003). The screening of the extracts was performed using our 3-D lymphangiogenic sprouting assay, measuring lymphatic sprouts as surrogate parameter for lymphangiogenesis. Stack-images and quantification were performed automatically. The positive hits were further validated using elaborate in vivo and ex vivo culture systems including RNAseq analysis. Additionally, clinical studies were performed to assess the effectiveness of formulations containing the extracts in vivo.

Results: With the lymphangiogenic sprouting assay we screened 2200 plant extracts and identified 5 hits, which in combination activated LECs by more than 270%. Culture of ageing LECs in the presence of this combination was found to induce the major lymphatic survival, maintenance and activity genes. Treatment of human skin explants visibly improved lymphatic morphology. A clinical study with the extract combination in a cosmetic formulation, conducted by an external provider, confirmed an improved lymphatic function through the significant reduction of periorbital edema.

Conclusion: Here we report the discovery of the first natural lymphatic activators, using a phenotype-based screening assay and a plant extract library. The identified hits synergistically induced lymphatic activation by more than 270% in vitro, coupled with an improved vascular phenotype and an induction of critical molecular pathways. The implementation of lymphatic activators in a clinical study using a cosmetic formulation led to significantly decreased periorbital puffiness and improve skin quality. Thus, lymphatic activators present a new dimension in functional skincare.

Objectives: - In vivo investigations of the restructuring effect on the skin barrier function and breathing effect of the new skin healing photoprotector
- In vivo analysis of the impact of the photoprotector on skin microbiome
- Clinical study after laser procedure to evaluate the tolerance and efficacy of the photoprotector in wound healing, especially on post-inflammatory hyperpigmentation

Introduction: The use of a photoprotector is commonly practiced following certain aesthetic procedures, in particular to prevent post-inflammatory hyperpigmentation (PIH). The objective of these studies was to demonstrate the tolerance and efficacy of a new skin healing SPF50+ photoprotector, formulated based on the ecobiological approach which considers the skin as a living ecosystem interfacing with its internal and external environment, and preserving its natural biology. Thus, four experimental studies targeting the biological mechanisms of the skin and one clinical study were carried out.

Materials / method: The protective and breathing effects of the photoprotector were investigated via two TEWL studies and by measuring the transcutaneous O2 and CO2 partial pressures (radiometer). Then, skin microbiome was analyzed via 16S rRNA gene sequencing after disinfection. Finally, the tolerance and efficacy of the product (twice/day, 14 days) were evaluated in 32 subjects after post-laser in a non-comparative clinical study. An overall scar score (3 subscores: inflammation, scar appearance, functional signs) was performed by a dermatologist in addition to instrumental measurements (TEWL, Visia).

Results: A significant improvement of the TEWL was observed after blotting paper (21.4%) and after rubbing (7.8%), without variation in transcutaneous O2 and CO2 pressures. The microbiome analysis showed a diversity restoration from 3hrs after disinfection, unlike the control. Finally, in addition to no adverse event, the photoprotector significantly reduced the overall scar score by -13.7% immediately after laser up to -57.1% at D14 (in particular PIH -21.8%) vs D0. Moreover, a significant reduction of the pigmentation (-17.9%) was observed vs D0 and of the TEWL (-6.5% from D1 vs after laser).

Conclusion: In conclusion, this new skin healing SPF50+ photoprotector designed using an ecobiological approach, promotes the natural healing process while protecting against UV rays, in the respect of the skin's ecosystem, especially its microbiome, for optimal healing without hyperpigmentation.

Objectives: Rejuvenation is often thought of on a MACRO scale, but the most important thing happens on a MICRO scale.
Mitochondria are unique cell organelles responsible for energy production. They play an important role in the regeneration of the body particularly through induction of
programmed cell death (apoptosis) and regulation of the redox state of the cell. Mitochondrial dysfunction leads to many diseases and premature aging. Supporting the energy centers of our cells is the role of the so-called mitoceuticals, which are widely available as dietary supplements as the prevention of aging.

Introduction: Dietary supplements with a potential anti-aging solution that work at the mitochondrial level include strong antioxidant glutathione, its precursor - N-acetylcholine (NAC), and nicotinamide adenine dinucleotide (NAD) boosters like nicotinamide mononucleotide (NMN) and nicotinamide riboside (NR). These substances are available without a medical prescription and usually dosed according to online instructions. Without an understanding of the processes taking place in mitochondria with the participation of these substances, the possibilities of effectively and safely treatment are reduced.

Materials / method: The analysis of available medical research on glutathione, NAC, NMN and other substances that increase level of NAD+ in the cell aims to understand the effects of these supplements, determine their safe doses and indicate potential side effects. Opinions of leading doctors involved in longevity research were also included.

Results: Numerous studies have proven that glutathione and its precursor - NAC protect the cell against the effects of free radicals and reduce their amount. NAD+ precursors increase energy in the cell. In this way, these substances counteract aging, old age diseases and cancer. Unfortunately, there have also been reports of the so-called paradoxicall effect of these extremely beneficial substances in the case of cancer. Studies on mice have shown that increasing cancer cell energy and reducing oxidative stress in cancer cells contributes to their proliferation and metastasis.

Conclusion: The latest research suggests that glutathione, NAC, NMN, NAD+ can counteract many age-related diseases and prevent the development of cancer. However, they should be used in a limited manner, as some studies suggest that they may have adverse effects in cancer. It seems that due to their strong effects, they should be even classified as medicines. Before we find the answer how these substances work in human bodies, supplementation with glutathione, NAC, NMN, NAD+ boosters should be discussed with the doctor, especially if the patient has or had cancer or has a family history of cancer.

Objectives: In this cross-sectional study, we aimed to assess stigma related to pigmentary skin diseases across 34 different countries.

Introduction: Pigmentary disorders are a group of skin diseases characterized by changes in skin tone. These diseases can occur at any age, affecting men, women and children of all ethnic backgrounds. They can have a significant impact on the perception of physical health, aesthetic appearance towards patients and deeply affect quality of life. To date, no study has specifically addressed the stigma experienced by individuals with pigmentary disorders.
The PUSH-D, a skin specific patient reported outcome assessing stigma, was recently published.

Materials / method: In each participating country, the sample was representative in terms of age and sex of the adult population. Self-diagnosis questionnaires allowed us to identify patients who declared pigmentary disease confirmed by a physician. Socio-demographic data and the type and localization of pigmentary disease were retrieved for each participant who were also asked to answer the PUSH-D. The PUSH-D, was recently published. It is composed of 17 items, easily understandable regardless of the socio-cultural level of the respondents and has been translated and validated in more than 20 languages.

Results: 48,000 individuals were recruited who declared having been diagnosed by a physician with a single PD (n=12332). 48.9% declared having SL, 7% Melasma, 3.3% Vitiligo, 15.7% isolated axillary hyperpigmentation, 11.3% PIH, and 11.2% POH. Mean PUSH-D score was 8.2±12.3 in men vs 9.3±13 in women; with significant difference according to age: younger subjects showed higher PUSH-D scores. No significant difference was found according to phototype. Whereas this difference becomes significant in patients with PIH and POH. Highest PUSH-D stigma score were found in vitiligo patients and melasma patients.

Conclusion: A better knowledge on the populations that feel stigmatized should be taken into account in dermatoses management. However, to date and to the best of our knowledge, stigmatization in dermatology has not been studied in skin diseases in a large population. This may be due to the lack of skin specific tools that explore different domains of stigmatization. The PUSH-D score may fill this gap as it explores both enacted and felt stigma.
