Objectives: After attending this lecture, attendees will be exposed to the latest developments in combination treatments to improve pathological scars using 1064-nm fractional picosecond lasers according to the LIOB technique in association with other innovative strategies

Introduction: Fractional 1064-nm picosecond lasers can be successfully used to re-modulate scar tissue concentrating photoacoustic effects within dermal layers preserving epidermis when LIOB technique is used.

Conclusion: The innovative LIOB technique confirmed its innovative approach to successfully soften pathological scars such as keloids and hypertrophic alterations avoiding photo-thermal tissue effects

Objectives: Scar treatment depends upon age and type of scar.
For traumatic and surgical scars, non-invasive measures to improve scarring include: silicone gel, massage, intralesional steroids. For keloids, options include intralesional steroids/5FU, topical imiquimod, cryosurgery, excision and radiation therapy. For acne scarring: retinoids, microneedling, chemical peels/CROSS, subcision, and punch excision. Optimal treatment often involves lasers and energy-based devices.
In the future we may see more exosomes, stem cells, and novel wound dressings. Early intervention is key!

Objectives: Surgery has an important place in the dermatologic field for diagnosis, treatments, restoration and sometimes beautification
Successful surgery is a priority, but it’s also important to optimize final scar appearance
Multiple components are to be, including:
- Hygienic and antiseptic measures before, during and after the surgery
- Technical considerations: good light, surgical material, characteristics of the incision, hemostasis, optimal type of wound closure

Objectives: Laser technology has been used to improve the quality of skin for more than 35 years however, selective thermolysis was a dominating approach in the field; one wavelength for one absorptive target. Sequential laser radiation, different wavelength laser applications, have been used over the last 15 years but without great acceptance by the medical and aesthetic communities.

Introduction: The authors shall present a new laser that can be defined as an advanced system in the medical and aesthetic technology markets.

The authors shall present the use of an innovative laser with two wavelengths that can also be used independently and simultaneously at any possible ratio of 1550 and 1927 nanometers, which differs from equipment already on the market. The laser system has high energy pulses similar to the existing large-size lasers but is reduced to pocket size (198x70x52 mm) weighing 330g, and operated from a rechargeable battery connected to the device via cable

Materials / method: Evaluation of dimensions of photo-thermolysis zones (collagen transformation) of human front abdomen skin after a non-ablative fractional laser radiation: MultiFrax simultaneous radiation of two beams at 1550nm and 1927nm and MultiFrax consequent radiation at the same at 1550nm and 1927nm at the similar pulse energy.

Front low part abdominal soft, normal color, elastic skin was chosen for the study following by resection. Symmetric left and right areas were chosen for the comparative laser irradiation. Areas were divided to nine of 3-4 cm squares.

Results: The cumulative tissue effect of simultaneous beams of different wavelength laser radiation to the skin demonstrated a higher and broader energy deposition under the surface of the skin. Collagen transformation at the depth of the skin zone (the “stem”) is due to the increased energy penetration of 1550 nm wavelengths.
Due to the cumulative effect, a deeper energy penetration of 1550nm wavelength radiation a collagen transformation was observed at the simultaneous irradiation compared with sequential irradiation of the skin at similar fluences of laser pulses.

Conclusion: Authors recommend the use of this novel multibeam simultaneous laser technology in dermatology, plastic surgery and aesthetic medicine to treat various lesions such as postsurgical and burn hypertrophic scars, acne scars, melasma, age spots, rhytids, stretch marks, poikiloderma.

Materials / method: Intralesional chemotherapy and contact cryotherapy have emerged as promising yet underutilized alternatives in the management of keloids. Intralesional chemotherapy, employing agents such as 5-fluorouracil, vincristine, and docetaxel, directly targets fibroblast proliferation and collagen synthesis, providing a mechanism-driven approach to reducing keloid size and alleviating symptoms. Recent clinical studies have shown high response rates in appropriately selected patients, with significantly lower recurrence rates compared to traditional therapies.

Results: Contact cryotherapy results in cold-induced tissue destruction. Unlike traditional spray cryotherapy, contact cryotherapy achieves uniform tissue freezing while sparing surrounding healthy skin, leading to improved clinical outcomes. Cryotherapy not only reduces keloid volume effectively but also enhances symptom relief and prevents recurrence when used as a monotherapy or in combination with other modalities.

Conclusion: This presentation will review the evidence supporting the efficacy and safety of these innovative treatment modalities, highlighting their potential to transform the current paradigm of keloid management. By leveraging the unique mechanisms of intralesional chemotherapy and the precision of contact cryotherapy, clinicians can achieve superior outcomes, reduce recurrence rates, and improve the quality of life for patients with keloids.
