Objectives: To investgate the upper eyelid depression's anatomy formation and its causes, treatment Introduction: Upper lid depression is very common on east asian's aging population and with double eyelid plasty or blepharoplasty surgeries popular in China , the iatrogenic upper eyelid depression can be seen post-operation as complications. To know the anatomy of upper eyeid depression is very important and can guide the treatment properly. Materials / method: Set group A with 10 aging people 65y in average , group B with 10 adults who had iatrogenic upper eyelid depression 25y in average Results: The anatomy formation of aging group A have orbital septum fat compartments and ROOF hypoplasia , the orbicularis flap and septum fat and ROOF augmentation with septum palpebralis adhesion release and upper lid resection have good results . The anatomy formation of adults group B have surgical defect of orbital septum fat compartment and ROOF, the adhesion between septum fat and palpebralis also common in these cases and the reconstruction of orbital fat compartments and ROOF can have good results Conclusion: The orbital septum fat compartment and ROOF augmentation are important to treat the upper lid depression in aging population and surgical defect after blepharoplasty.
