Introduction: Although the application of fat grafting to the breast is still a controversial issue, hybrid techniques in which breast prosthesis and fat grafting are applied together are quite popular. However, the aim of hybrid approach is deep fat application to increase the volume of the breast. One of the biggest problems in breast aesthetic/reconstructive patients is deterioration in skin quality, sagging and striae formation. Therefore, this study presents the application of fat with a different technique to improve the quality of the skin tissue of the breast. Materials / method: In addition to standard fat applications, a 400 micron size fat graft was also applied to the subdermal and dermal areas in a total of 21 cases who underwent aesthetic/reconstructive breast surgery for different reasons, aiming to improve the quality of the breast skin.For this purpose, the fat tissue harvested with a special cannula was centrifuged with a 500 g centrifuge for 2 minutes and lipocondensed, then reduced to 400 micron size using ultra-sharp blades and applied to the breast skin on all foreheads in subdermal and dermal plan. Results: The patients were followed for a minimum of 6 and a maximum of 3 years, and the change in the appearance and quality of the breast skin was recorded, and the evaluation was made by VAS by the patients and independent surgeons. No complications related to the procedure were observed in any of the patients. Conclusion: The most important difference that distinguishes aesthetic and plastic surgery from all other surgical branches is that the result obtained is visually perfect as well as functional. If this is actually a gift presented to the patient, it is very important that it is gift wrapped. In order to achieve the desired result in breast surgery, skin quality must also be improved in addition to volume. Application of 400 micron fat graft with the ARAT (adjustable regenerative adipose transfer) technique ensures the improvement of the breast skin, which has become striae due to weight loss.
