Objectives: 1.Understanding the severe grade gynecomastia management with out excising the skin 2. Use of quilting sutures in severe grade gynecomastia 3. Seroma Management 4. Technique of NAC Plaster Lift Introduction: Severe grade gynecomastia is one of the difficult problem to handle. Our clients does not want to have visible scars in the chest. Especially darker skin individuals don’t want to go for double incision mastectomy and free nipple graft because of the visible scars. So we have come up with various modifications to give reasonably good results with very minimal scars. Materials / method: We have used Ultrasound and power assisted liposuction followed by complete gland removal. After the gland removal we used extensive quilting to avoid seroma as well as managing the skin redundancy. Once the incision was closed , We have used plaster lift technique to redistribute the extra skin . we have done 70 cases in last one year. Results: With this above method, We have avoided double incision mastectomy scars and give a reasonably good results. Conclusion: In severe grade gynecomastia, using extensive quilting sutures and plaster lift technique gives a reasonably good results.
