Toward Personalized Facial Feminization: Simulation and Innovative Medical Device
Objectives: To evaluate the contribution of digital simulation and patient-specific medical devices in facial feminization surgery (FFS), with the aim of optimizing precision, safety, reproducibility, and aesthetic outcomes.
Introduction: FFS encompasses a set of cranio-maxillofacial procedures targeting both bone structures and soft tissues. Its primary goal is to reduce gender incongruence and improve quality of life. Advances in three-dimensional imaging and personalized manufacturing now allow for predictive and individualized planning, while enhancing surgical safety.
Materials / method: • Population: patients undergoing FFS for gender incongruence at Henri Mondor University Hospital.
• Planning: high-resolution imaging, three-dimensional morphological simulation, and predictive analysis.
• Innovative devices: patient-specific cutting guides and custom-made implants manufactured by 3D printing.
• Procedures: forehead reconstruction, orbital contouring, rhinoplasty, genioplasty, and gonial angle reshaping, adapted to each patient’s individual characteristics.
Results: • Preoperative simulations improved surgeon–patient communication and refined the definition of surgical goals.
• Patient-specific devices increased surgical accuracy, reduced operative time, and enhanced intra- and postoperative safety.
• Outcomes were reproducible and harmonious, with high patient satisfaction and a significant reduction in gender dysphoria.
Conclusion: FFS is transitioning from standardized procedures to a personalized, technology-assisted approach. The integration of 3D simulation and innovative medical devices improves precision, safety, and predictability of results, reinforcing FFS as a medically necessary intervention and contributing to the psychological and social well-being of transgender patients.
