Combined therapeutic approaches in the management of acne scars
Objectives: Acne scars cause long lasting physical and emotional burden. Beyond visible skin damage, patients experience loss of confidence, social withdrawal, and anxiety. Early and combined therapeutic strategies are essential to treat active acne, prevent scarring, and improve quality of life. This study aims to evaluate the effectiveness of a multimodal approach integrating medical, injectable, and energy-based treatments for atrophic and hypertrophic scars.
Introduction: Acne scars create permanent physical marks and deep emotional impact. Beyond visible irregularities, patients often experience low self-esteem, anxiety, and social withdrawal. Early intervention is essential not only to control inflammation, but also to prevent irreversible scarring. Combining medical treatment (topical or systemic) with regenerative and energy-based techniques offers a comprehensive approach to improve skin quality and quality of life.
Materials / method: Patients with active acne or visible scars were managed with a personalized multimodal protocol. Depending on scar type and severity, treatments included: acne control (topical/ oral therapy), subcision or scar release, microneedling or RF microneedling, TCA CROSS, chemical peels, fractional lasers, intralesional corticosteroids for hypertrophic scars, and soft-tissue fillers (hyaluronic acid or calcium hydroxyapatite) for volumetric improvement.
Results: Patients treated with the multimodal protocol showed significant improvement in scar depth, texture, and global skin quality. Subcision combined with RF microneedling or TCA CROSS yielded rapid reduction of atrophic scars, while intralesional corticosteroids effectively flattened hypertrophic lesions. Fillers (HA or CaHA) improved contour and self-image. Patient satisfaction increased markedly, with decreased psychosocial distress and fewer requests for corrective procedures.
Conclusion: A combined, stepwise strategy offers superior clinical outcomes compared with single modality treatment. Addressing both the inflammatory phase of acne and the scar maturation phase is essential to avoid permanent damage. Multimodal protocols improve texture, depth, and skin quality while significantly reducing psychosocial distress. Early intervention,treating acne and scars simultaneously,provides the most natural and long-lasting results.
