Vascular lasers: endpoints and complications
Objectives: To describe the adequate endpoints treating vascular lesion and to avoid complications
Introduction: Vascular lasers are a cornerstone in the treatment of a wide spectrum of dermatologic conditions, including vascular malformations, telangiectasias, rosacea, poikiloderma, and hemangiomas. Clinical success and safety largely depend on the correct identification and interpretation of immediate treatment endpoints, which guide energy delivery while minimizing adverse events.
We review and illustrate the key clinical and dermoscopic endpoints associated with vascular laser treatments, and to analyze common complications, their mechanisms, warning signs, and practical strategies for prevention.
Materials / method: This presentation integrates current evidence with extensive clinical experience using pulsed dye laser (PDL), Nd:YAG 1064 nm, KTP 532 nm, alexandrite, IPL, and combined laser approaches. Immediate and early endpoints are correlated with laser parameters (wavelength, pulse duration, fluence, spot size, and cooling), vessel characteristics (depth, size, density), and skin phototype. Clinical photography and dermoscopy are used to document treatment responses and to differentiate therapeutic from warning endpoints.
Results: Therapeutic endpoints such as purpura, transient vessel darkening, and vessel disappearance are shown to correlate with effective vascular targeting, while warning endpoints including whitening, blistering, epidermal damage, and excessive edema signal an increased risk of complications. Dermoscopy enhances pre- and post-treatment assessment, particularly in port-wine stains and complex vascular lesions. Common complications—such as post-inflammatory hyperpigmentation, ulceration, scarring, grooving, and demarcation lines—are analyzed with practical pearls to reduce risk
Conclusion: A thorough understanding of laser–tissue interaction and immediate clinical endpoints is essential for safe and effective vascular laser treatment. Recognizing warning signs early, individualizing parameters, and incorporating dermoscopic evaluation significantly improve outcomes while minimizing complications. Mastery of endpoints should be considered a fundamental skill in advanced laser dermatology.
