Laser for brusing and OCT: a story of settings
目标: Bruising and hematomas are frequent after aesthetic, surgical, or traumatic procedures and may significantly affect recovery and patient satisfaction. Although usually self-limiting, their visibility motivates strategies to accelerate clearance. Laser and IPL therapies target hemoglobin-derived chromophores involved in bruise evolution. Advanced imaging tools such as LC-OCT offer new opportunities to better understand bruise physiopathology and the tissue-level effects of light-based treatments.
介绍: This systematic review aims to synthesize current evidence on laser and IPL therapies for bruise and hematoma management. Secondary objectives include identifying optimal devices, parameters, and timing, while exploring how LC-OCT imaging may contribute to understanding vascular and erythrocyte changes induced by laser treatments.
材料 / 方法: A systematic PubMed search was conducted on November 9, 2025, using MeSH terms related to bruising and laser or IPL therapies. Seventeen studies were included. Extracted data included clinical context, device type, parameters, timing, and outcomes. In parallel, LC-OCT was used in clinical practice to explore superficial vascular changes associated with post-laser evolution.
结果: Seventeen studies across post-injection, post-surgical, traumatic, and senile purpura contexts demonstrated overall clinical benefit. Pulsed dye laser (585–595 nm), long-pulsed Nd:YAG (1064 nm), Q-switched and picoseconds lasers, and IPL systems all showed accelerated bruise resolution, particularly when applied 24–72 hours post-trauma. Outcomes varied due to heterogeneous protocols and study designs. LC-OCT consistently revealed an increase in superficial vessel diameter following laser treatment.
总结: Laser and IPL therapies represent effective, physiologically grounded approaches for bruise management. Pulsed dye laser appears optimal for early, superficial ecchymoses, whereas LP Nd:YAG should be better suited for deeper lesions and darker skin phototypes; IPL offers a versatile option for diffuse and multichromatic bruising. LC-OCT imaging demonstrated post-laser vessel dilation, suggesting that vascular modulation may contribute to accelerated bruise clearance. Despite encouraging clinical outcomes, protocol heterogeneity and limited evidence quality underscore the need for standardize
