From Volumization to Regeneration: The emerging role of biostimulators in modern aesthetic dermatology.
Objectives: This lecture will provide an in-depth presentation of the emerging role of biostimulators as regenerative agents in aesthetic medicine, highlighting their mechanisms of action, clinical applications, and evidence-based protocols for optimizing collagen induction, skin quality, and patient outcomes. It aims to equip practitioners with the knowledge and tools to effectively implement biostimulatory injectables into aesthetic practice.
Introduction: In recent years, aesthetic dermatology has undergone a remarkable shift from immediate volumization toward regenerative approaches focused on long-term dermal remodeling. Biostimulators such as Calcium Hydroxyapatite (CaHA), Poly-L-Lactic Acid (PLLA), Poly-D,L-Lactic Acid (PDLLA) and Polycaprolactone (PCL) represent a new generation of injectables that promote neocollagenesis, elastin synthesis, and extracellular matrix restoration. These agents are increasingly used to improve skin quality, texture, and firmness beyond simple volumetric correction.
Materials / method: A comprehensive literature review and demonstration of personal clinical experience was conducted, focusing on the pharmacological properties of various biostimulatory agents, multiple injection protocols, personalized indications, synergistic use with other treatments (energy based devices as well as PRP, Polynucleotides and exosomes), safety and complication management as well as presentation of clinical cases with 12 months follow-up.
Results: Biostimulatory treatments showed progressive and sustained improvements in dermal density, elasticity, and overall skin quality. Patient satisfaction was high due to natural-looking results and minimal downtime. Histological data confirmed increased Type I collagen production post-treatment. Treatment efficacy was enhanced when biostimulators were combined with synergistic modalities such as RF, microneedling, Polynucleotides and PRP. Proper product and patient selection played important role to successful outcomes and minimization of unwanted side effects.
Conclusion: Biostimulatory injectables are redefining the future of aesthetic dermatology offering new options for skin rejuvenation based on natural and long-lasting aesthetic outcomes. Their successful integration into clinical practice requires a shift in mindset — from volumizing to regenerating structure and skin quality. This lecture will provide practical, science-based strategies to safely and effectively incorporate biostimulatory injectables into daily practice.
