Posterior Ligament Suspension and Jawline contouring
Objectives: To describe a novel thread-lifting technique targeting posterior ligament suspension for jawline contouring, illustrated through clinical cases. The aim is to present the anatomical rationale, planning principles, and aesthetic outcomes, and to discuss how this vector can be applied alone or combined with midface lifting to enhance contour definition, improve lower face harmony, and prolong lifting effects while maintaining a favorable safety profile.
Introduction: Thread lifting is widely used for lower face rejuvenation, yet recurrent sagging often occurs due to insufficient support from posterior mandibular ligament structures. Addressing this anatomical zone with barbed suspension threads with a modified technique may create a more stable anchoring point, optimizing the lifting vector and reinforcing jawline definition. Combining posterior and midface vectors can provide synergistic effects, improving contour, balance, and durability of results in selected patients.
Materials / method: Clinical cases were selected to illustrate the use of bi-directional barbed suspension threads (measuring 500mm in length, double-edged needle) for posterior ligament support aiming to improve jawline contour. The approach consists of creating a vertical vector along the posterior mandibular region to reinforce this anchoring point, and may be combined with midface lifting when indicated. Each case was documented through standardized photographs and clinical assessments, focusing on contour definition, symmetry, and treatment safety during follow-up.
Results: Treated cases showed consistent improvement in jawline definition, reduction of jowling, and improved lower face contour. When combined with midface lifting, outcomes appeared more aesthetically balanced, with smoother transitions between the mid and lower thirds. Patients reported high satisfaction and natural results, with no serious adverse events.
Conclusion: Posterior ligament suspension and jawline contouring with barbed threads appears to be a safe and reproducible technique that enhances jawline contour while reinforcing fixed ligament area and helps prevent recurrent sagging. The clinical cases illustrate its potential both as an isolated approach and as a complementary vector to midface lifting, offering synergistic effects, improved lower face harmony, and results, making it a valuable addition to thread lifting strategies for facial rejuvenation.
