Facial Nerve Paresis Treatment using PLA/PCL threads
Objectives: To evaluate the efficacy of PLA/PCL thread implantation as a minimally invasive treatment for facial nerve paresis, addressing both functional impairment as well as aesthetic asymmetry.
Introduction: Facial palsy results from loss of facial nerve function at any point along its course, leading to impaired facial expression, oral incompetence, and disfiguring asymmetry. This condition significantly impacts quality of life due to both functional disability and psychosocial distress. Current management strategies include pharmacological therapy, botulinum injections, and surgical procedures; however, these may provide limited or temporary results, or carry higher morbidity. Minimally invasive thread lifting offers a novel alternative that combines mechanical support with tissue regeneration.
Materials / method: A 71-year-old female with right-sided facial paresis since 2003, without focal neurological deficits, presented with involuntary periocular spasms, persistent oral drooling, and aesthetic asymmetry. The patient had previously received repeated botulinum toxin injections with minimal benefit. PLA/PCL threads were implanted to restore facial symmetry, improve periocular and perioral control, and address both functional and aesthetic concerns while avoiding surgical intervention.
Results: PLA/PCL thread implantation achieved significant correction of facial asymmetry and improvement of functional symptoms, including reduction in drooling and better periocular muscle control. Compared with prior non-invasive treatments, the outcomes were superior and more durable. Results were comparable to those expected from more invasive surgical techniques, but achieved with minimal downtime and no major complications.
Conclusion: PLA/PCL thread lifting is a safe, effective, and minimally invasive option for patients with facial nerve paresis who wish to avoid surgery. This technique provides simultaneous functional and aesthetic benefits, with excellent patient satisfaction and minimal recovery time, representing a valuable addition to the therapeutic armamentarium for facial palsy management.
