Objectives: One of the most severe complications in aesthetic medicine is embolism. And as practice shows, even the use of cannulas often does not provide protection. To properly manage this condition, anatomical knowledge is crucial. After all, the affected area is not always located at the site of the injection. Introduction: The facial vessels have a vast number of anastomoses, through which an embolus can reach distant regions. Therefore, it is necessary to inject the antidote not only at the injection site but also in the zones where the embolus could have traveled with the bloodstream. This is why knowledge of these anatomical aspects is so important. Materials / method: I would like to present several interesting clinical cases. For example, necrosis in the occipital area after injections in the temporal zone, necrosis in the infraorbital region after injections in the nasolabial fold, and necrosis of the tongue after injections in the chin. In all of these cases, the anatomical reasoning is clear, due to the presence of anastomoses—knowledge of which is essential and which I would like to discuss today. Results: I would also like to present successful cases of treating occlusions after injections of hyaluronic acid and calcium hydroxyapatite, using not only classical methods but also hyperbaric oxygen therapy and medical leeches.
