Objectives: understanding the special anatomy of the periorbital area ( upper and lower lid, medial and lateral canthus, orbital bone structure etc) understanding the criteria for selecting the appropriate flap to close local defects in the periorbital region essential tips and technical pearls for periorbital reconstruction Introduction: The complex anatomy of the periorbital area presents a constant challenge for reconstruction, especially in skin cancer surgery with local flaps. Materials / method: In a retrospective study of 400 patients who underwent surgery between 2013 and 2025 we analyze the aesthetic result and function after reconstructive surgery of the nose, concerning the different localization of the cancer and the chosen local flaps of reconstruction. Complications and special difficulties in this type of surgery are discussed. Results: Glabellar transposition flap for le medial canthus area, temporal cheek rotation flap ( Mutardé) for reconstruction of the lower lower lid , transposition and advancement flaps for the upper lid are our preferred and mostly used local flaps Conclusion: Being aware of the pitfalls of the periorbital area, respecting the aesthetic units and accounting for the complex anatomy of this complex region we propose guidelines to achieve optimal form and function in reconstructive perorbital surgery .
