Beyond Wrinkles: Off-Label Use of Botulinum Toxin Type A in Non-Surgical Gluteal Lift and Reshaping
Objectives: Learning Objectives
Upon completion of this presentation, participants will be able to:
1. Demonstrate the anatomical landmarks and injection techniques for safe and effective gluteal botulinum toxin type A (BoNT-A) administration (Skill-based).
2. Evaluate patient candidacy for gluteal BoNT-A lifting by analyzing indications/contraindications and comparing outcomes with alternative procedures (Clinical decision-making).
3. Implement evidence-based BoNT-A dosing protocols (50–100 U per side) to achieve predictable contouring while minimizing adverse effects (Practical application)
Introduction: The growing demand for minimally invasive gluteal enhancement procedures necessitates exploration of innovative techniques. While botulinum toxin type A (BoNT-A) is well-established for facial wrinkles and hyperhidrosis, its potential for gluteal remodeling remains understudied.
Materials / method: This study enrolled 16 patients presenting with mild gluteal ptosis or asymmetry. Intramuscular injections of botulinum toxin type A (50-100 units per side) were administered into the gluteus maximus muscle using a standardized quadrant injection protocol. Outcomes were assessed at 1-, 4-, 6-month follow-ups utilizing:
- Standardized photographic documentation
- Global Aesthetic Improvement Scale (GAIS)
- Gluteal projection angle measurements (lateral view analysis)
Results: The study demonstrated significant clinical outcomes:
- 93.75% (n=15/16) of patients exhibited measurable improvement in gluteal contour with mild lifting effect (p<0.01)
- Mean effect duration: 4.5 months (±0.7 months)
- Adverse events:
- Transient muscle weakness: 6.25% (n=1)
- No cases of gait disturbance or severe complications
- Patient-reported outcomes:
- 81.3% satisfaction rate (GAIS ≥3)
- Significant improvement in body image perception (p=0.02).
Conclusion: This pilot study demonstrates that botulinum toxin type A (BoNT-A) represents a promising minimally invasive approach for gluteal contouring, particularly in patients with:
- Mild-to-moderate ptosis (Grade I-II)
- Muscular asymmetry (≤20% volume discrepancy)
