Novel Intervention for the treatment of burn scar alopecia
Objectives: To study the effect of combination of nanofat grafting and hair transplantation in treating burn scar alopecia.
Introduction: Treating burn scar alopecia remains a challenging problem for hair transplant surgeons. Several factors, including tissue stiffness, poor tissue and poor blood circulation, reduce the success of hair graft survival in scar tissue. Nanofat grafting is an effective technique not only for promoting the regeneration process but also improving the mechanical structure of scar tissue. It offers a potentially effective regenerative option to treat burn scar. It increases the success of hair transplantation in burn scar alopecia.
Materials / method: In this study, 15 patients of burn scar alopecia of 18 to 55 years age group were treated with combination techniques. The procedures were conducted in two stages: First, the nanofat grafting; second, hair transplantation. After one-month, nanofat grafting improve scars and angiogenesis. Then, hair transplantation was performed with Follicular Unit Extraction (FUE). Assessment were evaluated with photography and trichoscopy at baseline, 1,3 and 12 months.
Results: Twelve months after hair transplantation, it was successfully performed with excellent results. The survival rate of transplanted follicular unit is 85 -90 %, and the density success rate per square centimeter is 70 -75%. I achieved hair density of 25 – 30 grafts per cm² in burn scar region. Patients expressed high satisfaction with the result and no complication was encountered.
Conclusion: In case of burn scar, the skin of the scar is poorly supplied with blood, so that hair follicles cannot survive, and hair growth does not take place. The combination technique, with prior nanofat grafting, is effective for burn scar alopecia. It contains growth factors to promote hair growth. With its ability to enhance cell communication, modulate gene expression and promote a healthy scalp environment. This enhances the effectiveness of the regenerative process, allowing for more successful hair transplantation outcomes.
