Microlifting and PCL threads
Objectives: Describe the technique of upper and lower micro-lifting using PCL threads with barbs.
Demonstrate how deep plane traction combined with gallia anchoring improves the stability of the repositioning.
Present a less invasive, outpatient alternative with rapid recovery, indicated for mild to moderate laxity.
Discuss expected clinical results, safety points, limitations, and possible complications.
Introduction: The search for facial rejuvenation with quick recovery and less surgical complexity has increased, especially among younger patients with mild to moderate sagging and a desire to improve the contour of the jawline, neck, and mid-face, without hospitalization and without the need for a more complex surgical center.
In this scenario, PCL (polycaprolactone) barbed thread lifts are gaining ground because they are bioabsorbable and, in clinical practice, offer a longer support time compared to PDO threads, in addition to stimulating a tissue response with progressive improvement in skin quality.
Materials / method: PCL traction wires with barbs (traction/anchoring wires).
Nylon wires for gallia anchoring.
Ambulatory instruments for wire placement and plane preparation.
Materials for local anesthetic infiltration, antisepsis, and dressing.
Standardized preoperative marking for traction vectors and anchoring points.
Standardized photographic documentation (frontal, oblique, and profile) for comparison of results.
Results: The technique aims to deliver natural results, with noticeable aesthetic improvement and preservation of individual features. The main expected results are:
In the lower Micro Lifting:
More defined jawline.
Improvement of cervical laxity and neck contour in mild to moderate cases.
Reduction of the "drooping" appearance of the lower third of the face, with improved face-neck transition.
Cleaner finish in the peri-auricular region due to limited skin resection.
In the upper Micro Lifting:
Improvement of the middle third of the face, with malar repositioning in selected patients.
Conclusion: Micro Lifting with PCL threads is an outpatient technique that combines deep plane traction, anchoring in gallia with nylon, and limited skin resection, offering a minimally invasive alternative for patients, especially young patients, with mild to moderate sagging.
The approach allows for improvement in mandibular contour, cervical sagging, and, in selected cases, mid-face laxity, with short scars and faster recovery.
