Facial fat reposition - MADA technique
Objectives: With age, facial fat is lost, tissues dehydrate, collagen decreases, increasing the effects of aging. In the quest to improve aesthetic results in facial fat repositioning, anatomical knowledge of the compartments to be filled is crucial, as well as the attachment of the transferred tissue. The goal of the MADA technique is to minimize contamination risks and ensure cell viability, ensuring a result with low complication rates and high resolution benefits.
Introduction: Given the high demand for facial volumization products, fat is a very noble tissue to handle. If it can be extracted and processed safely for placement, that's why in my learning curve, I optimized with the MADA technique not only the quality of the fatty tissue I transfer but also the shaping treatment of the extraction area, offering patients multiple aesthetic treatments in a single medical act.
Materials / method: - Antiseptics
- Sterile fields
- 500cc physiological solution
- IV set (perfus)
- 30 and 21 gauge needles
- 1 mm x 15 cm Klein cannula
- 2% Lidocaine with epinephrine 40ml
- 1/1000 Epinephrine ampoule
- 3.5 mm blunt tip thick cannula
- 3-way stopcock
- Transfer devices
- Multiple fat filters (nanofatgrass)
- Syringe holder
- 1.5mm diameter x 7cm long blunt tip needle with 1 hole
- 60, 20, 5, and 1cc syringes
- Sterile gloves, gauze, and adhesive tape
- Pressotherapy
- 3 MHz ultrasound
Results: Replacing fatty tissue in facial compartments creates a natural and visually pleasing sensation, not only volumizing but also producing restorative effects on skin aesthetics due to stem cell remodeling over time, along with post-surgical collagen stimulation.
Conclusion: With knowledge of the anatomy of the area to be treated and the use of standardized protocols like MADA (closed, reproducible, with a medium learning curve) and aesthetic concepts adapted to each patient, this practice yields extraordinary results as an autologous product that volumizes with regenerative cellular effect due to the high percentage of stem cells in fatty tissue.
