Pain management in mesotherapy: a prospective randomized controlled trial comparing ring block, syringe, mesogun and topical anaesthetic cream
Objectives: The aim of this half-head split study is to determine whether the use of a mesogun device reduces pain and discomfort during mesotherapy drug administration and potentially eliminates the need for local anesthesia.
Introduction: Intralesional therapies in aesthetic medicine are increasingly important, especially for hair loss, offering notable benefits. However, pain during mesotherapy injections is a major drawback and often leads patients to stop treatment early, even though optimal results require multiple sessions. While ring block with local anesthesia can help, it is uncomfortable, causes downtime, and is demanding for physicians. Efficient pain management is crucial to improve patient adherence to treatment protocols.
Materials / method: Forty-two patients with androgenetic alopecia received PRP, dutasteride or biclutamide. Each participant had split half-side injections: one side was treated with a standard 5 ml syringe, 31-gauge, 4 mm needle, 0.05 ml single shot at 4 mm depth; the other side used a mesogun with the sameinjection parameters. Pain was measured on a verbal numerical scale (1–10) and evaluated by the physician. Subgroups included numbing cream for both devices. After four weeks, all patients received a ring block for comparison both sessions with comfort rated on a scale from 1 (excellent) to 6 (unsatisfactory).
Results: Significant differences in pain scores were observed between mesogun and syringe groups, with the greatest reduction in pain seen in the mesogun with numbing cream subgroup. Overall, the mesogun demonstrated superior pain management compared to the standard syringe, especially in sessions without local anesthesia.
When faced with decision between ring block and mesogun, all patients favored the mesogun.opt for it. With a conventional syringe, 36% opt for ring block; with numbing cream and syringe, 24% do. The average comfort rating for the mesogun was 1.7, compared to 3,4 for the ring block.
Conclusion: Mesotherapy with a mesogun and numbing cream provides better pain relief and comfort for androgenetic alopecia than traditional syringe or ring block methods, making ring block unnecessary.
These findings suggest that adopting mesogun technology can enhance patient tolerance and adherence to mesotherapy protocols, ultimately improving clinical outcomes in aesthetic medicine.
