From weight loss to body recomposition: preventing sarcopenic weight loss with a hybrid nutrition and HIFEM protocol
Objectives: To evaluate a structured clinical protocol integrating personalized medical nutrition therapy and HIFEM technology in preventing sarcopenic weight loss during medical slimming, and to demonstrate its role in achieving body recomposition through fat reduction with preservation of skeletal muscle mass.
Introduction: Conventional weight loss approaches primarily target reduction in body weight, often without preserving skeletal muscle mass. This can result in sarcopenic weight loss, which negatively impacts metabolic health and aesthetic outcomes such as muscle definition and body contour. A paradigm shift toward body recomposition is required, emphasizing fat reduction with preservation of skeletal muscle mass to optimize both clinical and aesthetic outcomes.
Materials / method: A clinical case series was conducted in overweight male patients undergoing a structured medical slimming program. The intervention combined personalized medical nutrition therapy with controlled energy intake (28–30 kcal/kgBW), protein intake (1.5–2.0 g/kgBW), and micronutrient optimization, with scheduled HIFEM sessions targeting abdominal muscles. Body composition was assessed before and after the intervention. Muscle strength was evaluated using 1RM testing. Standardized clinical photography was performed to document changes in body contour and muscle definition.
Results: Both cases demonstrated reductions in body weight, fat mass, body fat percentage, and visceral fat following the intervention. Despite sustained caloric deficit, skeletal muscle mass was preserved or modestly increased. Case 1 showed improved muscle mass alongside fat reduction, while Case 2 achieved significant fat loss without meaningful muscle loss. Strength performance improved in both cases, supporting preservation of muscle function. Clinical photography demonstrated visible improvement in abdominal contour and muscle definition, supporting the body recomposition effect.
Conclusion: Medical slimming should prioritize body recomposition rather than weight reduction alone. A hybrid approach combining personalized nutrition and HIFEM technology may effectively prevent sarcopenic weight loss while promoting fat reduction and preserving skeletal muscle mass. This integrated strategy supports improved metabolic health and enhances aesthetic outcomes, highlighting the essential role of muscle-preserving interventions in modern aesthetic medicine.
