Secretomes - the new kid on the block
Objectives: To define secretomes and their biological role in regenerative signalling; understand paracrine mechanisms in tissue repair and rejuvenation; differentiate secretomes from stem cell therapies; review current clinical applications in aesthetic and dermatologic medicine; and evaluate safety, regulatory, and translational considerations.
Introduction: Secretomes—bioactive factors released by mesenchymal and other progenitor cells—represent a cell-free regenerative strategy. Comprising growth factors, cytokines, peptides, and extracellular vesicles including exosomes, secretomes mediate tissue repair through paracrine signalling. They offer potential advantages over live-cell therapies, including improved safety profiles and simplified clinical application.
Materials / method: This presentation reviews molecular components of secretomes, mechanisms of action in collagen synthesis, angiogenesis, immunomodulation, and anti-inflammatory pathways. Clinical protocols in skin rejuvenation, hair restoration, wound healing, and combination treatments with PRP and energy-based devices are discussed, alongside manufacturing standards and regulatory frameworks.
Results
Results: Emerging clinical evidence suggests secretome-based therapies may enhance dermal regeneration, improve skin texture and elasticity, accelerate healing, and reduce inflammatory responses. Early comparative data indicate promising outcomes with reduced procedural risk compared to cell-based approaches, though standardisation and long-term studies remain necessary.
Conclusion: Secretomes represent a significant advancement in regenerative and aesthetic medicine, shifting focus from cellular transplantation to biologically active signalling. With ongoing research and appropriate regulation, secretome therapies may redefine minimally invasive regeneration and future aesthetic practice.
