Herbal remedies for androgenetic alopecia: saw palmetto- friend or foe?
Objectives: After this session, participants will be able to review the pathophysiology of androgenetic alopecia, focusing on the role of 5α-reductase and dihydrotestosterone in follicular miniaturization; explain the biological mechanisms of action of Serenoa repens and other evidence-based herbal therapies supported by human in vivo studies; and analyze the clinical efficacy and safety of pure topical Saw Palmetto extract using objective and subjective assessments.
Introduction: Androgenetic alopecia (AGA) is a widespread hair loss condition in men causing significant psychosocial distress. While standard medications work, side effects drive interest in plant-based alternatives. This study evaluates the clinical evidence for various herbal remedies and specifically tests the efficacy and safety of a pure topical Serenoa repens (Saw Palmetto) extract as a natural treatment option for men with AGA.
Materials / method: This study had two phases. First, a structured literature review identified herbal treatments for androgenetic alopecia (AGA), focusing on outcomes, mechanisms, and evidence quality. Second, a prospective study included 40 men aged 30–50 years with mild to moderate AGA. Participants applied pure topical Saw Palmetto extract twice daily for 12 weeks. Efficacy was assessed by target area hair counts, blinded global photographic assessment, and a 7-point satisfaction score. Adverse events were monitored.
Results: The literature review found several herbal agents for AGA, mainly with antiandrogenic or anti-inflammatory effects, but evidence was limited by small samples and heterogeneous designs. In the clinical study, Serenoa repens significantly increased total and terminal hair counts and reduced vellus hair counts after 12 weeks (p < 0.001). Most participants showed visible improvement on photographs, and satisfaction scores increased. No adverse events were reported.
Conclusion: Evidence for herbal therapies in AGA remains limited due to methodological weaknesses. However, this study demonstrates that pure topical Serenoa repens extract is effective and well tolerated in men with AGA. These findings support Saw Palmetto as a potential treatment option and justify larger controlled trials to confirm its efficacy and compare it with standard therapies.
