Precision intervention and long‑term management strategies for melasma as a chronic disease based on multidimensional pathogenesis
目标: Currently, a wide array of treatment modalities exists for melasma; however, short‑term pigment improvement is no longer the primary clinical challenge. Rather, low therapeutic efficacy and long‑term recurrence are the core unresolved issues in clinical practice. Our approach overcomes the limitations of traditional single‑modality therapies, significantly enhances clinical satisfaction, effectively reduces long‑term relapse rates, and provides a novel clinical paradigm for the standardized management of melasma in Asian women of reproductive age.
介绍: Melasma, a chronic facial hyperpigmentation from photoaging, affects 30% of Asian childbearing‑age women. Its variable response and high recurrence challenge treatment. Contrary to myth, it is controllable like hypertension. Though photodamage persists, systematic therapy can achieve complete clearance and stability. Pathogenesis involves barrier disruption, inflammation, photoaging, and vascular changes, not just melanin. Monotherapies like peels or lasers targeting only melanin often worsen inflammation and recurrence.
材料 / 方法: Based on this multidimensional pathogenesis, we have established a comprehensive diagnostic and therapeutic system for melasma. It targets and metabolizes formed abnormal melanin for rapid improvement, while also restoring barrier function, controlling chronic inflammation, and improving photoaging and the microenvironment, thereby blocking melanogenesis activation and aberrant transfer at the source. Additionally, individual factors such as UV exposure and endocrine changes are addressed for long‑term maintenance and stability.
结果: Clinical satisfaction with melasma treatment has improved, and the long‑term relapse rate has been reduced.
总结: This integrated model overcomes the limitations of traditional single‑modality treatments and offers a novel clinical pathway for the standardized management of melasma in Asian women of reproductive age.
