Non-surgical management of severe open wounds: clinical outcomes in bed sore–like lesions
目标: To demonstrate the clinical efficacy of exosome-based regenerative protocols in treating complex, stage III/IV chronic open wounds (bed sore-like lesions) and to propose a non-surgical alternative for patients with high surgical comorbidities.
介绍: Chronic ischemic wounds and pressure ulcers often reach a "stagnation phase" where conventional wound care and surgical grafting fail due to poor vascularity or systemic patient frailty. This presentation explores how the immunomodulatory and angiogenic potential of exosomes can restart the healing clock in necrotic microenvironments, offering a potent alternative to invasive reconstruction.
材料 / 方法: A retrospective observational study was conducted on patients with chronic, non-healing "bed sore-type" lesions. The protocol utilized:
Active Agent: High-purity, Salmon-derived exosomes (selected for their potent anti-inflammatory and regenerative signaling).
Delivery: Targeted delivery into the wound bed and perilesional margins.
Evaluation: Healing was quantified via the rate of granulation tissue density, neo-angiogenesis markers, and volumetric reduction of wound cavities.
结果: The integration of exosome therapy led to a significant "Angiogenic Switch" within the wound bed.
Key outcomes included:
Accelerated Granulation: Rapid filling of deep cavities with healthy, vascularized tissue.
Bio-debridement Effect: Reduction in inflammatory exudates and necrotic stagnation.
Epithelial Advancement: Remarkable wound margin contraction even in cases previously scheduled for flap surgery.
No systemic adverse events were recorded, and patient recovery time was reduced by approximately 40% compared to traditional secondary intention healing.
总结: Exosome-based therapy effectively bridges the gap between palliative wound care and invasive surgery. By modulating the chronic inflammatory environment and promoting microvascular regeneration, this approach provides a predictable, non-surgical roadmap for managing the most challenging open wounds in aesthetic and reconstructive practice.
