Objectives: To systematize and develop algorithms for the correction of postoperative deformations of the mammary glands by the type of "waterfall"; to characterize key surgical techniques for the correction of various types of postoperative deformations of the mammary glands by the type of "waterfall" according to the developed algorhithms.

Introduction: In modern plastic surgery, one of the most popular and frequently performed operations is augmentation mammoplasty (breast enlargement) - as of 2023, more than 10 million women around the world have breast implants.
However, despite the use of modern technologies, the results of breast augmentation plasty are not always satisfactory. Quite often, after the implant is installed, especially in combination with mastopexy in the postoperative period, its deformation and various postoperative complications occur.

Materials / method: 197 patients with secondary postoperative breast deformities were examined and operated taking into account the proposed classification of deformities. Observation of patients in order to assess the recurrence of deformity and identify its causes was carried out for 18 months.

Results: The division of postoperative deformations "waterfall" (snoopy breast) into 4 types, where the main characteristics and key surgical techniques of correction are described.

Conclusion: Thus, plastic operations on the breast in the form of augmentation mammoplasty with mastopexy can be complicated by the development of postoperative and delayed deformations such as "waterfall" .
The proposed algorithms for correcting the types of deformations facilitates the planning of the secondary operation, outlines the phasing in the correction procedure.
Understanding the problem allows the surgeon to plan an appropriate corrective operation and minimize secondary postoperative aesthetic complications.

Objectives: Demonstrate how fat is used in aesthetic breast surgery

Introduction: Fat grafting to the breast has opened up new options and refinements in aesthetic breast surgery as well as solutions to an accumulating group of women who desire implant removal.

Materials / method: Analysis of 165 consecutive patients is presented and fat grafting is demonstrated in patients desiring primary natural breast augmentation, in patients desiring removal of implants, in place of implants in mastopexy/augmentation, in conjunction with implants as a composite technique and in a number of congenital conditions affecting the breast.

Results: Results have overall been excellent with high degrees of patient satisfaction. Complications specifically related to fat grafting have been rare with 2 cases of lipid cysts that required drainage and one case of delayed hematoma following an injury to the breast after fat grafting. There has been no incidence of infection of the fat or implant, and 2 cases of minor wound drainage that cleared with local wound care and oral antibiotics.

Conclusion: Fat grafting to the breasts is a great option for many patients desiring breast enhancement as well as those needing secondary management. Indeed, fat makes breast more beautiful.
