Objectives: The objective of this paper is to evaluate the predictability and safety of use of chonchal cartilage graft in correcting the post SMR deformity.
Introduction: we still come across many post SMR deformities. few of the ENT surgeon, prefer to do SMR for DNS, as oppose to septoplasty. These deformities are very typical, with depression of the tip area due to over harvest of the septum. These deformities look very awkward and one need to get it corrected.
These are two options for such deformity, 1) Chonchal cartilage graft and 2) Costal Cartilage grafts.
the chonchal cartilage is very simple with minimal donor morbidity or scar visibility.
Materials / method: 60 operations have been done done in 8 years using the conchal cartilage as the spreader graft, all procedures were performed in Islamabad cosmetic surgery center. All surgeries were done with an open tip rhinoplasty step ladder infra cartilaginous incision technique.
Results: 55 patients were quite satisfied with the outcome. 03 patients were not very happy with the results they thought they will get. 02 patients were happy but I was not so happy. All 60 Patients had satisfactory results. All surgeries were evaluated based on the pre operative and post operative photograph by an independent plastic surgeon. The results were classed from excellent to poor based on the post operative pictures showing the improved achieved in different areas.
Conclusion: Conchal Cartilage is easy to harvest and have minimal donor area morbidity. One can get at least two conchal cartilage and the predictability and reliability is very good. As oppose to rib cartilage graft or bone graft this give you a good pliable cartilage with minimal or no donor deformity. The scar is totally concealed behind the ear and there in minimal to no pain as compare to costal cartilage donor morbidity and pain.
