Objectives: 1. Retain youth features and manage the aging process of the face and neck with minimal scar.
2. No scar Power Assisted face and neck Lifting for patients with mild to moderate skin excess.
Short scar Power Assisted face Lifting for patients with moderate to important skin excess.
3. Achieve stability of long term results with minimal complications.

Introduction: The author presents his experience with a new concept of Power Assisted face and neck Lifting based on limited subcutaneous undermining, wide tunnelization, SMAS plication, and closed platysmoplasty with VLOC loops, without neck undermining.

Materials / method: Following tumescent infiltration, Power-Assisted Liposuction (PAL) and wide subcutaneous and SMAS/platysma tunnelization were done using a 3-mm custom made blunt cannula. Liposuction of the submental and “jowls” fat excess was performed when needed.
Limited undermining, plication of the SMAS using Vloc 0 and resection of the excess skin were performed through a preauricular incision. Neck lifting was performed using VLOC 0 suture, placed in a loop fashion to tighten platysmal bands and better define the mandibular angle without undermining the neck. Lipofilling was performed when required.

Results: The technique was applied for 200 patients. Age ranged between 40 and 75. Smoking and redo face lift patient rates were 38.4 % and 23.1 % respectively. All the patients were discharged on the same day following the surgery. There were no recorded hematomas, infections nor neurological deficits. Patient satisfaction was rated as high and the follow up ranged between 2 and 5 years.

Conclusion: Stability of the aesthetic shape is ensured by the improved laxity of the SMAS following tunnelization and its plication using loops. Long term result is maintained by progressive fibrosis. The loop tightens and approximates platysmal bands. The technique has several advantages including reduced operative time, reduced complication rate as well as the possibility of combining it with lipofilling. It constitutes a good indication for smoking patients, and recurrent skin laxity following previous facelift, with sustained results at long term follow ups.

Objectives: We present our experience technique in the last 5 years for autologous dorsal nasal augmentation rhinoplasty using diced cartilage in an autologous carrier wrapped / free (in a sleeve of fascia grafts)

Introduction: .This is a modification or evolution of the classical Diced Cartilage Grat,-DCG+Deep Temporal Fascia –DTF- complex where NO DTF is used. The degree of resorption of the diced cartilage wrapped in fascia was found to be considerable compared with 1-piece block grafts, most likely because of histologic evidence of fibrosis and inflammatory cell infiltration.

Materials / method: Cartilage was diced into 0.5-1 mm fragments depending on differents types of skin . PRP was obtained by blood centrifugation 1800 rpm 8 minutes. PRPG is used as scaffold , then we place DCG mixed with PRP and we cover with PRPR. Closed approach is used in 90% on cases

Results: Follow up for the last 5/6 years.2 videos showing the biology and the technique will be available.This technique is predictable, what you place is what you obtain 95%, no skelotonization.

Conclusion: Another tool for the large armamentarium for rhinoplasty operations , primary & secondary.

Objectives: Learn to harvest fat from the upper limb when lipofilling is applied to the hand.

Introduction: In this short movie, the authors present their technique of lipofilling applied to the hand and in particular as an adjunctive procedure to the surgical management of Dupuytren’s contracture. The technique highlights a novel donor site, fully adapted to the hand as a recipient site, with minimal invasivity and morbitdity. The authors also present adapted equipment for swift, minimally manipulated and guideline-complying harvesting of the adipose tissue.

Objectives: The new compact Q-graft® system provides the possibility to combine the harvest of adipose tissue with the isolation and concentration of SVF cells in one closed sterile environment in a standardized manner.

Introduction: Adipose tissue is a known source of stromal vascular fraction (SVF) cells. They are a promising tool for regenerative medicine and tissue engineering. Today laboratories have established many differing manual SVF-isolation procedures. These methods rely on laminar flow benches to ensure a sterile working environment. In order to be able to consecutively harvest adipose tissue and isolate the SVF in the operating theatre during one operation, a different solution is necessary.

Materials / method: In combination with a body-jet the adipose tissue can be suctioned right into the Q-graft® system. Here it is digested with Humanase™, a blend of proteolytic GMP grade enzymes, under constant mixing and heating to 37 °C. Through a series of steps of washing and filtration the SVF cells can be collected as a suspension in Ringer’s solution. The suspension of the SVF can either be concentrated in the device by cross-flow filtration or by one step of centrifugation using a special sterile Q-graft® centrifugation set.

Results: The Q-graft® system is able to thoroughly separate the SVF cells from the adipocyte fraction of the adipose tissue. Preliminary results show, that with just one step of centrifugation the Q-graft® system was able to isolate an average of 36.3E+04 SVF cells per ml processed tissue. An average of 17.4 % of the cells reached plastic adherence and were CD34-positive.

Conclusion: No bacterial contamination was found in the tested cell suspensions. In ongoing work the CFU content, population doubling time of the isolated adMSC, as well as their mesenchymal differentiation potential are being determined. Thus, the successful isolation of the SVF in the closed Q-graft® system can clear the way for the clinical application of this regenerative cell fraction.
