Objectives: To objective the tolerance, and the reasons of some disappointing results using the cryolipolysis device (Coolsculpting) since 2009.

Introduction: Cryolipolysis, used since 2009, is now became a very common non invasive procedure to reduce undesired limited fat bulges on the abdomen, the back, the thighs, the arms, and the chin and the idiopathic gynecomastia.
More and more (several billions) patients have been treated with a high level of satisfaction, but, with time, we observe logically some side effects and disappointing results.

Materials / method: Our experience is based on the treatments (more than 4000) we made since 2009 at the Centre Laser International de la Peau de Paris.

Results: The main side effects are the risks of paradoxical hyperplasia, probably very uncommon (may be an incidence of 1/1000 ?), and more severe, but observed only with some devices,( never seen with our device, the CoolSculpting) the risk of necrosis of the skin.
Some patients present a moderate pain lasting a few days or weeks, and , very uncommon a great pain lasting a few weeks..
We will discuss those risks, and how to avoid and manage them.
The main warnings are related with the fact that some treatements are made on bad indications ; the consequence will be a disappointing result.

Conclusion: The profile of tolerance with our device is very good, wiyhout any risks of severe side effects. We must be respectful with the rules ; cryolipolysis is a safe and effective treatment for limited fat bulges, without a lot of laxity or any need of diet. If the fat hyperplasia is too diffuse, or if the fat bulge is too big, regarding the limited treated area, the treatment by cryolipolysis must not be planned.
Eventually, we have to discuss the surgical liposuction or the others non invasive procédures.

Objectives: To explain the role of Cryolipolysis as a modality for body sculpting and to analyse degree of satisfaction it delivers in the area of body shaping.

Introduction: Cryolipolysis is a new non-invasive, clinically proven method to reduce fat. It is established that subcutaneous fat cells are more vulnerable to the effects of cold than other surrounding tissues. This discovery, called "selective cryolipolysis," led to the development of the technology behind this non-invasive Procedure.
Lipids in fat cells crystalize at warmer temperatures than other water-containing cells. This is how Cryolipolysis targets only fat cells for destruction and spares other tissue such as skin, muscle and nerves. We used a US FDA Approved system which controls the procedure.

Materials / method: By freezing fat folds we can either debulk or sculpt them.The areas treated are abdomen(upper and lower), flanks, inner thighs,saddle bags, male breast ,arms,back (infrascapular and infra- axillary fat bulges ) and thighs.
Pre treatment photos are taken,visual and physical assessment was done.Resutrs were assessed at 1,2 and 3 months . patient satisfaction is assessed and is variable depending on pre condition and patient expectations.

Results: There are few Tips which have helped us to improve the treatment results-
-Do Good patient selection
-Do Realistic patient counselling
-Do Correct assessment
-Do not under treat
-Select Right applicator
-Correct positioning of applicator, and appropriate overlap
-Suggest the right post care-Wearing Corset,Exercise and healthy diet
-Repeat the procedure after2- 3 months if required
-Stacking of applicators over bulky areas has shown faster and better results

Conclusion: Cryolipolysis is an effective modality for body contouring and useful to reduce pockets of fat.It is not intended for weight loss or obesity. People seeking spot reduction for specific areas of fat and having modest size fat bulges are the ideal candidates for Cryolipolysis. This technology has high level of patient satisfaction.The results can improve with few tips and tricks .
