Objectives: HOT TOPICS IN CICATRICIAL ALOPECIAS

During the lecture, we will discuss 3 hot topics in cicatricial alopecias with direct application in clinical practice:

Introduction: 1. TREATMENT OF FRONTAL FIBROSING ALOPECIA WITH ANTIANDROGENIC DRUGS AND ORAL ISOTRETINOIN

-Oral antiandrogenic drugs (dutasteride and finasteride) are the treatments with a higher level of evidence in frontal fibrosing alopecia to achieve clinical stabilization.
-Oral isotretinoin is very useful to improve facial papules.

Materials / method: 2. NEW THERAPIES FOR LICHEN PLANOPILARIS

-Low dose oral naltrexone may be a potential adjunctive therapy for the treatment of lichen planopilaris (LPP).
-Low level light therapy has shown good clinical results in an experimental study of patients with LPP, improving the symptoms and increasing the hair density.
-Platelet rich plasma might be an interesting second-line therapeutic option in LPP patients.
-New clinical research about the potential usefulness of oral tofacitinib in patients with LPP has been recently published.

Results: 3. TREATMENT OUTCOMES IN A LONG-TERM CASE SERIES

-A multicenter series of 60 patients with long-term follow-up has been recently published in the Journal of the American Academy of Dermatology. The most effective therapy was the combination of rifampicin + clindamycin (300 mg every 12 hours for 10 weeks).
-Some non-antibiotic therapies may be considered: oral isotretinoin, photodynamic therapy, surgery.

Based on these new developments, we will show our proposal of therapeutic algorithms for each cicatricial alopecia.

Objectives: Now, three types of autologous fat grafts are at our disposal for reshaping and rejuvenation

Introduction: Liposuction was developed in the 1970s by Illouz, then the procedure of its careful injection of macrofat with the lipostructure of Sydney Coleman. Magalon has miniaturized it in microfat and has finally been transformed into nanofat in the hands of Tonnard and Verpaele

Materials / method: This evolution involved the one at each of the three stages of the procedure:
1. Harvesting the fat (diameter of cannula, hole size, etc.),
2. The treatment of fat (centrifugation, decantation, filtration ..)
3. And finally the fat transfer (from the cannula to the needle)

Results: The results are encouraging and sustainable

Conclusion: According to the previous wish:

- When: a lipotransfer procedure alone or accompanying a surgical lift or blepharoplasty

Objectives: Learn how to use HA fillers to improve results in periorbital rejuvenation

Introduction: HA fillers are one of the tools to restore volume in the periorbital region, nevertheless fine tuning and smooth transition in the lower eyelid cheek junction is not always simple. I present a systematic approach to get improvement even in small irregularities

Materials / method: HA Fillers
Lidocaine 2% with Epinephrine

Results: During the last ten years I have been using this approach to restore the softness to the lower eyelid cheek junction.
Diluted HA spread small particles within the tissues that improve also que skin quality probably because hydration.
The therapeutic effect last more than expected

Conclusion: HA fillers are a great tool to improve even small irregularities but care must be taken to avoid, additional irregularities, lumps, Tyndal effect or residual bags

Objectives: Hybrid Lifting

Purpose: Volume loss in facial adipose tissue plays a critical role in aesthetic of facial aging. Furthermore, the facial adipose system is a complex network of distinct compartments, and a detailed understanding of these compartments is essential for optimal facial volume restoration.

Introduction: Incorporation of selective fat compartment volume restoration along with superficial musculoaponeurotic system manipulation allows for improved control in recontouring while addressing one of the key problems in facial aging:the volume deflation.

Materials / method: This theory was evaluated by assessing the contour changes from simultaneous face “lifting” and “filling” through fat compartment facial fat transfer.

Results: The combination of face-lift or midface lift (Lift and Fill theory) with fat grafting, provides a reliable and reproducible aesthetic rejuvenation of the face especially in the middle third

Conclusion: The combination of different surgical techniques seems to be efficient in the problem solving of the anti aging surgery of the face

Objectives: The authors illustrate, all the diagnostic and laser technical solutions that can facilitate treatment with laser systems in subjects of any skin colour minimizing the risk of side effects, such as PIH, keeping the same efficacy profile.

Introduction: Nowadays the knowledge of the patient racial origin, and that one of parents and grandparents, should be always matched with a detailed “skin imaging report” to determine appropriate laser treatment modalities.

Materials / method: Indo-Pakistanis, Africans and Asians, Aboriginals and Mullatoes patients have a higher tendency to develop hypo and above all post-inflammatory hyperpigmentation (PIH) then Europeans, Nordics and Mediterraneans. The treatment of these potentially dangerous candidates requires a management by expert hands, opportune instruments and proper parameters .
The high performance 3D imaging systems provide life-like high quality 3D reconstruction images. This system includes an image management software with superior photo documentation capabilities to visualize, analyse and archive 3D images.

Results: With the 3D Analysis module, we can now detect and precisely quantify subtle changes quicky and easily about roughness measurements, volumes, 3D surfaces, pigmentations and vascular components.
Moreover the possibility to achieve remarkable results with a very low risk of side effects can depend on the features of the laser device employed.

Conclusion: The new generation Alexandrite laser, equipped with a special 7 mm handpiece integrated with a cooling system sapphire, turned out to be a promising and safe technique for the removal of cutaneous pigmentations.
