Objectives: To review causes of facial hyperpigmentation other than melasma in dark skinned persons.

Introduction: Melasma is a common pigmentory disorder of face, particularly in dark skin females. Several other less common diseases can also induce facial hyperpigmentation on the face in dark skin persons.

Materials / method: They include Riehl melanosis (pigmented cosmetic dermatitis), poikiloderma of Civatte (erythromelanosis interfollicularis), erythromelanosis follicularis faciei et colli, peribuccal pigmentation of Brocq, Berloque dermatitis, erythema dyschromicum perstans, lichen planus pigmentosus, and drug‐induced hyperpigmentation.

Results: A brief description of clinical features, pathogenesis, diagnosis, and treatment of these entities will be presented.

Conclusion: Several less common causes of facial hyperpigmentation should be considered in the differential diagnosis of melasma.

Introduction: Sharing How To Handle this difficult condition: A Dermatologists Approach and Perspective
Deramatologist's gold standard treatment for melasma and hyperpigmentation-
Discussing triple combination topical therapy.
New treatments for melasma and hyperpigmentation-
the use of second-line topical lightening agents like azelaic acid.
I like using 15-20% azelaic acid during the off-hydroquinone period.
Tranexamic acid can be given orally, and it can be given between 250 mg to 500 mg twice a day with excellent outcomes.

Materials / method: 1) Triple Combination Therapy
2) Topical Nutraceuticals
3) In office Photography

Results: Significant Improvement in skin color, pigment, and scars.

Conclusion: Using appropriate customized protocols and a compliant patient, hyperpigmentation may be treated effectively.
