YOUNG DERM AWARDS - Treatment of keloids by intralesional infiltrations of bleomycin: long-term follow-up

Objectives: The objective of this study was to evaluate the immediate (3 months) and long-term (24 months) efficacy of intralesional bleomycin injections (BIL) for the treatment of keloids and to estimate their tolerance.

Introduction: Keloids are a pathological healing process usually occurring after skin trauma, sometimes minor. Despite their benign nature, keloids can have significant repercussions on the quality of life of patients. The treatments vary in effectiveness and are rarely satisfactory. The literature is poor in therapeutic trials or retrospective series.
Bleomycin is a chemotherapeutic agent in the family of cytotoxic antibiotics.The few published studies to assess the efficacy and safety of bleomycin in keloids involved small samples of patients followed on short periods.

Materials / method: This retrospective, monocentric study (CHU of Reims), was conducted from January 2008 to December 2015, included all patients with keloid treated with BIL, with a minimum follow-up of 3 months after the last injection. Injections were performed by the same operator (10mL bleomycin ; concentration 1.5 IU/mL). The number of injections was adapted to the clinical response. Main outcome was the reduction of keloid size classified as: complete (100%), partial major (75% -100%), partial minor (50% -75%) and insufficient (<50%).

Results: Among 70 patients, 66 have been analyzed. This treatment required an average of 3.4 ± 1.3 injections per keloid. The efficacy of a 3 months-lasting treatment, evaluated among 57 patients, was greater than 75% for two thirds of the patients and completed for 40% of them. At 24 months, the evaluation was concerning 44 patients, 52% of them had a keloid decrease of more than 75% and for 30% of them, the recession was complete. Nine recurrences were observed among 50 patients (recurrence rate of 18%) over a follow-up period of 24.8 ± 10.5 months. No systemic complications have been occured.

Conclusion: Our study shows the short-term and long-term efficacy and safety of BIL in the treatment of keloids.

YOUNG DERM AWARDS - Comparative efficacy of lasers for the treatment of acne vulgaris: a systematic review and network meta-analysis

Objectives: 1: Comparing laser devices and its application on Acne Vulgaris.
2: Evaluate and Rank the various Lasers to assess the best therapy for Acne Vulgaris

Introduction: Acne is one of the most common disorders encountered in everyday practice. Lasers in particular have a central role in the management of acne, which tends to be refractory to medical therapies, while there is a controversy about which of laser is effective compared to others. Therefore, we aimed to evaluate and rank the various therapies through this network meta-analysis of clinical studies.

Materials / method: We searched on PubMed, Scopus, and Cochrane Central Register of Controlled Trials until June 2021, to include randomized controlled trials for acne vulgaris that compared lasers. A random-effect model of network meta-analysis with frequentist framework was performed for all treatments compared to Control. Mean differences (MDs) of lesions count with 95% confidence interval were calculated, and all interventions were ranked by the Surface Under the Cumulative Ranking (SUCRA) values.

Results: 18 RCTs were enrolled to assess the reduction of inflammatory and non-inflammatory lesion counts. Ranking probability showed that Erbium Glass Laser (SUCRA score= 0.90) and PDT+MAL (SUCRA score= 0.68) were significantly superior to control. Erbium Glass Laser was superior in treating inflammatory acne lesions compared to other lasers such as PDL and PDT+ALA. On the other hand, Erbium Glass Laser (SUCRA = 0.74), and PDT+ ALA (SUCRA score = 0.71) were significantly superior to control in reducing non-inflammatory lesions, while no significant differences were observed among other interventions.

Conclusion: This network meta-analysis demonstrated that lasers could be a good alternative treatment for those who were diagnosed with acne vulgaris. Erbium Glass laser and Photodynamic therapy showed a high probability of being effective for acne vulgaris.

YOUNG DERM AWARDS - Impact of microwave-thermolysis energy levels on patient-reported outcome measures for axillary hyperhidrosis and osmidrosis

Objectives: • Acquire knowledge on differences between energy levels of microwave-thermolysis device
• Understand the impact of patient-reported outcomes on patient satisfaction following microwave-thermolysis
• Support clinical decision-making for future treatment with microwave-thermolysis

Introduction: Microwave-thermolysis (MWT) is an emerging treatment for axillary hyperhidrosis reducing both sweat and odor. No prior studies have investigated and compared the different energy settings of the available MWT-device. This study evaluates patient-reported outcomes measures (PROMs) for axillary hyperhidrosis and osmidrosis following MWT-treatment at two different energy levels.

Materials / method: Twenty adults with axillary hyperhidrosis and osmidrosis reported sweat on Hyperhidrosis Disease Severity Scale (HDSS: 1-4) and odor on Odor Scale (OS: 1-10), respectively, supplemented by overall Dermatologic Life Quality Index (DLQI: 0-30). This was a prospective, randomized, patient-blinded and intra-individually controlled study with three months follow-up (FU). Randomization comprised MWT-treatment of one axilla with the standard medium energy setting (energy level 3) and the contralateral axilla with the standard high energy setting (energy level 5).

Results: At baseline, patients reported substantial sweat and odor, negatively affecting patients’ quality of life.
At 3-month FU, PROMs showed improved quality of life with significantly reduced sweat and odor. Overall DLQI was reduced from median 10 to 4, with a median reduction of 6.5 points (p=0.0002). HDSS was reduced from median 4 to 2 for both energy level 3 and 5, with a median reduction of 1 and 2 points, respectively (p=0.014). OS was reduced from median 8 to 3 for both energy levels with median reduction of 3.5 and 4 points for energy level 3 and 5, respectively (p=0.017).

Conclusion: Microwave-thermolysis effectively improved axillary sweat and odor three months after baseline. Correspondingly, patients’ quality of life improved significantly. Treatment with energy level 5 presented a subtle, but significant increase of efficacy based on PROMs for both sweat and odor reduction.

YOUNG DERM AWARDS - Real-life evaluation of the treatment of actinic keratoses by textile photodynamic therapy

Objectives: - Evaluate the efficacy of the first textile PDT device, in the treatment of actinic keratosis, in comparison to conventional PDT
- Evaluate the tolerance of the textile PDT treatment, and the satisfaction of the patients

Introduction: Actinic keratoses (AK) are a common precancerous skin condition in dermatology practice. Photodynamic therapy (PDT) with ALA or MAL is an effective but painful treatment of fields of cancerization particularly when conventional illumination sources and irradiation rates are used. Two prior studies showed that illumination with textile PDT was not inferior to conventional PDT but induced less pain. A first medical device has recently been marketed with textile based lighting. We performed a real-life study to evaluate efficacy and tolerance of this device.

Materials / method: We carried out a single-center retrospective study. We collected data from patients treated with textile PDT with MAL for AKs localized on scalp and temples between November 2018 and November 2019. The primary endpoint was complete clearance rate (CR) at 3 months-follow up.

Results: Data of 39 patients were reviewed in the study, with a total of 417 AKs. The CR rate was 72.6 % (95 %CI 67.9-77.0) at 3 months-follow up and 67.5 % (95 %CI 61.2-73.3) at 6 months-follow up. The median pain felt during the session was 0 and there wasn't erythema after the session for 64.1 %.

Conclusion: Our real-life study confirms efficacy and safety of textile PDT device in the treatment of scalp and temples AKs, with excellent tolerance and minimal pain reported.

YOUNG DERM AWARDS - Laser-assisted topical treatment of nail disease: a pilot study and review of the literature

Objectives: • Become familiar with what the literature says about laser-assisted drug delivery in nail tissue
• Understand the risks and benefits of laser pretreatment in the topical management of nail disease
• Learn from our early-adopter experience with laser-assisted topical treatment of nail psoriasis

Introduction: Ablative fractional laser (AFL) pretreatment for topical therapy of nail disease is gaining ground. To investigate the safety and efficacy of this drug-device treatment approach, we conducted a randomized intra-patient controlled trial on laser-assisted Calcipotriol/Betamethasone Dipropionate (Cal/BD) foam treatment for psoriatic nail disease (PND) and provided an overview of the pertinent literature.

Materials / method: We included 11 patients with 144 psoriatic nails with an average NAPSI score (0-130) of 80. Half of each patient’s nails were randomized to receive AFL at baseline, week 6, and week 12. Patients were instructed to apply Cal/BD foam once daily for 24 weeks. Treatment response was monitored clinically supported with patient-reported outcome measures, optical coherence tomography (OCT), and patient-obtained photographs using a smartphone app.

For the systematic literature review, two investigators searched Pubmed for clinical trials on laser-assisted topical treatment of nail disease.

Results: Cal/BD achieved a reduction in NAPSI -68% (p=0.001), 60% decrease in quality of life burden, and a 43% reduction in OCT-assessed PND. AFL pretreatment achieved higher clinical (NAPSI -78%) and OCT-assessed (-46%) improvement than Cal/BD alone (NAPSI -58%, OCT -37%) but with no significant difference. Smartphone app images documented paronychia (n=1) and mild local skin reactions.

We identified 21 articles on AFL pretreatment for onychodystrophy (n=1), PND (n=3), and onychomycosis (n=17). The majority of studies (90.5%) concluded that AFL pretreatment is safe and effective.

Conclusion: While Cal/BD foam treatment substantially improved PND both with and without AFL, the available literature suggests that laser pretreatment could be a clinically relevant enhancement strategy for various types of topical therapies. Future studies on the impact of laser settings and choice of formulation may help optimize the efficacy of laser-assisted topical treatment of nail disease.

YOUNG DERM AWARDS: Skin hyperpigmentation index: a new objective method for automated quantification of skin hyperpigmentation

Objectives: To establish an unbiased, and fully-automated method of quantitively evaluating and documenting skin hyperpigmentation.

Introduction: Measurement of skin hyperpigmentation is clinically important in the assessment, follow up, and therapy of hyperpigmented lesions. To date, there are no reliable objective methods for measuring skin hyperpigmentation quantitatively. Here, we report a novel, practical, unbiased, and fully-automated method of quantitively evaluating and documenting skin hyperpigmentation, the “skin hyperpigmentation index” (SHI) .

Materials / method: The SHI is a program with a deconvolution of color and an image histogram profiling of brown pixel intensities for the automated evaluation of quantitative skin pigmentation. The SHI defines the ratio of two scores, namely hyperpigmented skin of the affected area and normal sun protected skin from the same patient.

Results: This results in a range from 1 (no pigmentation) to 4 (maximum pigmentation). For a practical evaluation of the score, a free SHI-calculator can be used online, which is accessible to all practitioners (https://shi.skinimageanalysis.com/).

Conclusion: The SHI is a fully automated method for the quantitative assessment of skin pigmentation which can easily be applied to any skin type with any type of hyperpigmentation. This allows a simple, fast and standardized quantification of skin hyperpigmentation and is useful for monitoring the progress and planning of whitening therapy.

YOUNG SURGEON AWARDS - A new focus on face paralysis with mini invasive methods

Objectives: The methods described in this review are used to develop minimally invasive non-surgical procedures for managing posttraumatic and paralytic face deformities. Following patients post treatment effects functional and facial symmetry improvement was observed.
Problems associated with a disability of closing the eye, mastication and speech difficulties have been reduced, while a clear improvement of facial symmetry and self-esteem have been seen in all cases.

Introduction: Facial paresis is a life changing condition associated with loss of facial muscle movement ability that can be permanent or temporary. The Human face is one of the most functionally, socially, and aesthetically important parts of the body. Modern approaches have their limitations, this is the reason for demand of less invasive procedures for overall functional and aesthetic improvement. Thread Methods can be a substitution to static surgical interventions, a mini-invasive procedure with long-lasting results which can improve functional and aesthetic problems associated with Facial paresis.

Materials / method: A 20-year observational study of patients who underwent correction of visual aesthetic manifestations of the paresis in different face zones by thread lifting method was analyzed.
Correction was performed at: one-sided brow ptosis, inferior eyelid edge, cheek soft tissues, angles of mouth and other parts of the face. The thread used in such patients was
P(LA/ CL)barbed threads, P(LA/ CL)Spiral threads, same barbed threads composed of non-absorbable material proline.

Results: Satisfactory facial symmetry, tissue elevation, and volume creation are obtained without surgical interventions. 90 % of patients evaluated their appearance as a more attractive version of per-procedural appearance. These results demonstrate that thread methods are mini-invasive procedures that treat cosmetic problems effectively with facial paresis. The following result lasts up to 2 years but due to muscular activity absence in patients with facial paresis results are preserved longer.

Conclusion: This review found consistent evidence of barbed threads placement as an effective treatment for improving quality of life in facial palsy patients, with minimal risk of complications, short procedural, and recovery time, and provide long-lasting functionally and aesthetically favorable results.

YOUNG SURGEON AWARDS - The role of polyurethane implants in breast reconstruction

Objectives: -to define the types of implant-based breast reconstruction
-to define the complications associated with implant-based breast reconstruction

Introduction: The gold standard of breast reconstruction remains the autologous reconstruction with a pedicled or a free flap. However, sometimes this is not an option either because microsurgery is not available or the patients prefer to avoid the morbidity associated with the donor site. Breast implants represent an alternative in such cases. The polyurethane implants have the advantage of lower rates of capsular contraction and lower risk of displacement/malposition permitting at the same time an aesthetic reconstruction – they reconstruct the footprint, the conus and reinforce the envelope

Materials / method: personal cases

Results: In delayed breast reconstruction they are employed in a hybrid fashion with a pedicled flap or fat grafting. A smaller volume is used, limiting the long-term complications. In LD+implant reconstruction, the risk of implant migration in the back is absent.
In direct to implant breast reconstruction, the polyurethane implants can be used pre-pectorally without the risk of skin breakdown and implant exposure. In skin reducing mastectomy a polyurethane implant with borosilicate micropsheres is even a better choice because it is 30% lighter, reducing thus considerably the tension on the skin flaps.

Conclusion: The polyrethane implants are versatile devices that come in various shapes and help the surgeon achieve an aesthetic breast reconstruction.

YOUNG SURGEON AWARDS - In-house positioning and cutting guide system for genioplasty surgery: step by step guide

Objectives: 1. Discuss the advantages of using surgical guides in facial cosmetic surgery.
2. Compare surgical guide’s manufacturing options (custom vs. in-house)
3. Step by step protocol: how to design in-house surgical guides for mentoplasty (main objective).

Introduction: Advances in 3D printing technology have the potential to significantly improve the workflow of facial cosmetic surgical planning. Custom positioning and surgical guides allow for intraoperative reproduction of pre-planned osteotomy cuts, which can result in greater surgical accuracy and patient safety while maintaining an acceptable cost-effectiveness ratio.

Materials / method: Three-dimensional planning models are visualized and segmented using open-source software (3D Slicer) from imported conventional computed tomography (CT) data. To design the desired osteotomy cuts, freeware (Autodesk Meshmixer) allows computerized manipulation of mandibular components and virtual surgical planning.

Individualized surgical guides, are fabricated in surgical guide resin with a resin 3D printer (Formlabs) and allows precise intraoperative positioning.The osteotomised segments are fixed using a pre-adapted plate with 2.0 no locking screws.

Results: The two main advantages of using in-house surgical guides are cost and manufacturing time savings. We have the high initial cost of the necessary equipment, the learning curve in the use of the required programs, and the time spent in the design to contend with.

A custom design plate and surgical guides for mentoplasty performed by a commercial company cost approximately € 1500 and have an average delivery time of 7 to 10 days. With in-house surgicsal guides, the total cost is reduced to around € 100 per case. The design and production times are less than 48 hours.

Conclusion: The use of in-house surgical guides seems a reliable and recommended alternative in mentoplasty surgery. It allows to determine the cuts with greater precision and in a predictable way to the surgical planning. It is also a cost effective option for saving surgical time.

The learning curve for developing expertise with planning software and printing settings is compensated by increased surgical predictability and decreased operating time, making this type of planning a valuable investment.

Treating psoriasis: in line with current guidelines

Objectives: a) Know currently available sources for information on psoriasis (most recent guidelines and reviews)
b) Know current treatment recommendations and underlying evidence on psoriasis

Introduction: Every year, new treatment options are available for the treatment of psoriasis. Keeping up with it all is a true challenge. Systematic reviews and guidelines can help for orientation and give guidance for specific clinical situations.

Materials / method: Extraction of key recommendations from current guidelines and systematic reviews.

Results: While topical treatments have seen little developments in the past, a total of 17 systemic treatments are licences in 2022 in Europe. Attempts to compare all treatments among other others by network metaanalysis show very good efficacy with regard to PASI 90 response for the newer treatment from the group of anti-Il23 and anti-Il 17 antibodies, with older treatments such as adalimumab and etanercept shower lower response rates.
Safety profiles and suitability in specific situations continue to shape with more experience with these drugs.

Conclusion: A trade-off between efficacy, safety and costs has to be consider to choose a treatment. Currently the more effective treatments tend to have a higher price in most countries.
