Objectives: Photoaging is characterised by premature ageing, produced mainly by sun exposure classically attributed to UVA radiation, accumulated throughout individual’s life. This phenomenon is added to the body’s own intrinsic ageing process and involves a series of physiological, morphological and structural changes in the skin.
Introduction: Photodamaged skin or dermatoheliosis referres to a group of clinical manifestations that are characterized by formation of atrophic skin, deep wrinkles, lentigines, yellow, leathery, cobblestone skin, telangiectasias, senile purpura, photoinduced dermatoses etc.. Histological changes include solar elastosis, collagen fragmentation, irregular and reduced epidermal and dermal thickness, increase of inflammatory markers, melanogenesis disorders.
Materials / method: We performed the treatment protocol over 18-weeks with 7 in-clinic-sessions followed by a specific home care programme. The clinical protocol consisted of mesotherapy, filler treatments with cross-linked HA and chemical peelings. The home treatment consisted of 1% retinol and moisturising/regenerating topicals including consistent and very high UV protection. The patient showed a high level of compliance which allowed optimal results.
Results: An improvement in the reduction of the depth of wrinkles was evidenced with 3D images, with a change of age calculation by VISIA from 74 years old before the treatment to 44 years after treatment. There was also an improvement in skin firmness of 27% in the malar area, a reduction in melanin of 17%, an improvement in WSRS/Glogau/GAIS/Fitzpatrick scales and “evident improvement” in researcher/patient efficacy questionnaires.
Conclusion: With this case of severe photo aging we showcase that a non-invasive and sustainable combination treatment protocol using injectables and chemical peels can be highly effective to safely turn back the clock on facial aging.
