Pressure alopecias: unveiling the impact of cosmetic procedures on hair loss

Objectives: This lecture aims to provide a comprehensive understanding of pressure alopecias in the context of cosmetic procedures. Specifically, the objectives are to: Describe the two primary types of pressure alopecias: type 1 induced by external pressure and type 2 induced by injectable cosmetic treatments. Explore the underlying pathogenesis, including the role of mechanical trauma, inflammation, and disruption of the hair growth cycle. Discuss the clinical presentation, diagnosis, and differential diagnosis of pressure alopecias, emphasizing the utility of trichoscopy and histopathology.

Introduction: Pressure-induced alopecias (PA) are an uncommon subtype of scarring and non-scarring alopecias that occur after pressure-induced obstruction of capillaries, resulting in hypoxia on the hair bulb that leads to circumscribed areas of hair loss. Recently, PA have been categorized into two primary types: type 1 PA, induced by external pressure, and type 2 PA, involving internal pressure applied to the skin, as in the context of injectable cosmetic treatments. Prompt diagnosis is paramount as it might avoid preventable complications.

Materials / method: To ensure a comprehensive coverage of the topic, a thorough literature search was conducted. Electronic databases, including PubMed, Scopus, and Google Scholar, were utilized to identify relevant articles and studies. The search was performed using appropriate keywords related to pressure-induced alopecias, such as "pressure-induced alopecias," "alopecia induced by external pressure," and "alopecia induced by injectable cosmetic treatments." The search was limited to articles published in the English language to ensure consistency in understanding and interpretation.

Results: Type 2 PA occurs when a substance is injected into the dermis or hypodermis, typically through cosmetic procedures. The primary mechanism believed to cause type 2 PA involves the direct pressure exerted on arterial vessels and capillaries by the injected substances. This pressure occurs in areas with limited flexibility, potentially leading to ischemia and/or vascular tamponade, even without direct blockage of the blood vessels. HA, calcium hydroxyapatite, mesotherapy, and autologous fat grafts have been documented as substances that have been associated with the occurrence of type 2 PA.

Conclusion: This lecture provides a comprehensive overview of pressure-induced alopecias (PA), focusing on type 1 PA induced by external pressure and type 2 PA induced by injectable cosmetic treatments. Type 2 PA is a relatively rare complication often associated with cosmetic injectables. The scalp, forehead, temporal areas, and beard are particularly vulnerable to developing alopecia in these cases. It is crucial for practitioners to be mindful of alopecia as a potential side effect following cosmetic skin infiltrations, with inadequate technique, high-risk areas, and volume being key risk factors.

RF microneedling in androgenic alopecia: my experience

Objectives: Alopecia is a common clinical complaint for patients and often a source of significant psychological distress. Many treatment modalities are available. Novel drug delivery approaches are needed to maximize results and minimize potential side effects

Introduction: Various trials have been conducted on the management of male and female pattern hair loss, but the outcomes often seem to be limited. Adjuvant therapies are urgently needed.

Materials / method: 16 patients were enrolled in this scalp trial. Participants received monotherapy with topical minoxidil twice daily to one half of the scalp, while on the other half of the scalp the treatment with twice-daily topical minoxidil was combined with five sessions of Radiofrequency (RF) microneedling at 4-week intervals. Mean hair count and hair thickness, global assessment by the investigators, subject self-assessment and adverse effects were assessed.

Results: After 5 months of treatment, mean hair count and hair thickness increased significantly on the combined-therapy side. No side effects were noticed.

Conclusion: Combined treatment with fractional radiofrequency microneedling, topical minoxidil and nano drug delivery particles could be an effective and safe treatment option for male pattern hair loss.

New treatment approaches for androgenetic alopecia and alopecia areata

Objectives: This provides an overview of treatment approaches for androgenetic alopecia and alopecia areata using new substances and combinations, contrasting and comparing them with existing therapies.

Introduction: It involves the introduction of growth factors with CE approval in combination with the application of a Thulium laser. A distinction and comparison are made with previous therapies such as PRP and mesotherapy, as well as with medical standard treatments like minoxidil or ß-estradiol.
In the area of alopecia areata, there is also an overview of various treatment options, including the use of Baricitinib. The speaker has years of experience in treating hair disorders and shares his personal experiences in comparison.

Materials / method: hair stimulation with growth factors, thulium laser and LED-light

Results: in comparison to minoxidil (improvement of about 17%) improvement of 60 to 70%

Conclusion: new combination of hair stimulation with CE approval and higher improvement rate than minoxidil or PRP

Nanofat and hair restoration

Objectives: The objective of nanofat is induce regeneration in treated scalp and hair at the molecular level and prevent shockloss hair after hair transplantation. Longhair transplantation is to safely transfer hair follicles from the donor area to the recipient area without shaving or trimming to obtain a natural-looking result and restore hair volume.

Introduction: Hair transplant is becoming the most common cosmetic procedure across the world with demand for higher density and coverage of baldness.
NS-FUE was introduced in 2016 and has the advantage of donor area preview. With no need for donor area shaving.
A possibility of postoperative donor hair effluvium is reported, which can happen post-surgery. The condition may appear after few weeks of the surgery and slow diffuse hair loss.
Nanofat can improve neovascularization and graft survival.It has been successfully used in several procedures in regenerative medicine.

Materials / method: We improved the nanofat processing for restore hair growth and treat different types of hair loss. A total of 30 patients (28 men and 2 women). Nanofat is maintain hair health before and after NS-FUE. Nanofat was processed via mechanical emulsification and filtration. Injecting into the scalp for female pattern hair loss, and were treated for male pattern baldness. They had Norwood Type III baldness to Type VII baldness. NS-FUE was applied after received nanofat two months.The total surgery time was approximately 5 hours.it harvest donor hair without shaving.The recipient area looked natural.

Results: Shock loss or recipient-site effluvium was not seen in patients who continued to have preexisting hair within the transplant zone. Nanofat helped in reducing the risk and early recovery. All patients showed natural hair transplant results at their follow-up visits and were satisfied with the results.
NS-FUE was the immediate visualization of the result at 1 year postoperatively. In addition, the recipient area looked natural and was camouflaged by long hair.
The patient's photographs were taken. The body hair with scalp has enhanced the visual density, coverd in all grades of baldness.

Conclusion: This finding confirmed that nanofat promoted hair growth, a higher number and density of hair. In each case shock loss can be prevented by using nanofat for anagen effluvium, telogen effluvium. Beyond that, combination method showed result in hair promotion. These result further supported nanofat and longhair transplantation (NS-FUE) are working in combination, leading to and additively effect.
Althogh I've found the beneficial effect of combination on hair growth. There're still existed some limitations. Firstly, the difference of age and hair morphogenesis should be further analyzed.

Exosomes, a ray of hope for hair loss patients? Its limitation, evidence, and future in hair regeneration

Objectives: Topical minoxidil/ peptides and other oral medications has been a disappointment in patients with Hairloss . Here at this stage , Exosomes becomes as an effective and targeted therapy in hair loss patients and in long term maintenance proves itself to a promising treatment.
In the paper presentation I would be discussing on the the molecular mechanism of action of Exosomes with data available for its use in hairloss patients , along with few review of case studies in context to Indian patients .

Introduction: Exosome based therapy was used with Microneedling in 20 patients with Male Androgenetic Alopecia and Female pattern alopecia which showed significant decrease in hairloss and efficacious improvement in hair texture and in regrowth to quite an extent.

Materials / method: 2 male patients and 8 female patients with different grades of Androgenetic alopecia and female pattern alopecia were treated with micro needling along with Exosomes formulation with 2 sessions at a gap of 3 months. Patients were assessed using Hair Pull test, Global photographic assessment and Physician and patient subjective assessment scale at baseline and 3 months.

Results: Significant reduction in hairfall was seen in all the patients. Hair texture improvement was self noticed by the patients in 9 male patients and 6 female patients. New fine hair regrowth was seen in 17 out of 20 total patients which was seen as early as in 4-6 weeks on average . Subjective scoring by the patients and a blinded Objective scoring was done by a Dermatologist to assess the results in all the patients which showed improvement in most of the cases. There was slight pain and erythema in 3 out of total 20 patients .

Conclusion: Limitations - There are few biological, technological and regulatory issues that needs to be addressed before using exosomes which would be discussed in my paper presentation.

Conclusion -Microneedling along with Exosomes formulation is an effective , convenient and easy to perform procedure in comparison to other cell based therapies or mesotherapies. The results were faster as compared to other therapies. It is definitely an efficacious therapy but more trials and studies are needed to validate the same.

Non-invasive laser smooth stimulation of hair growth

Objectives: The aim was to evaluate the effectiveness and safety of non-ablative Er:YAG laser, new stmooth protocol used in clinical practice, alone or in combination with other treatment modalities, in patients with both early and advanced stages of AGA.

Introduction: Laser hair growth stimulation is an effective method of treating hair loss disorders. This innovative solution is focused on stimulation dormant hair follicles to promote hair growth. With New Laser Smooth modality enables treatments that ensure highly controlled delivery of energy and provide the best comfort for the patient. Practitioners can effectively and safely deliver unique temporal and spatial temperature profiles to the scalp, resulting in increased tissue regeneration and the promotion of new follicle growth.

Materials / method: We used this new protocol in 90 patients
The method of action lies in the smooth pulses that gently deliver laser light to the tissue, triggering the scalp in a safe and controlled way. Laser Smooth operates at the optimal infrared Er:YAG laser wavelength (2.94 μm) that coincides with the maximal absorption in the skin.
Efficacy was assessed with clinical assessment of AGA grade (Ludwig scale for female / Norwood-Hamilton scale for male)

Results: We had demostrated high efficacy and an absence of adverse effects. Significant improvement of hair growth along with increased hair density, and a stabilization of hair loss were reported, regardless of AGA (androgenetic alopecia) grade, patient age or AGA duration.

Conclusion: Our results suggest that the non-ablative Er:YAG laser new smooth mode as a monotherapy is an efficient and safe treatment for AGA—with a high satisfaction rate among patients regardless of patient age, AGA duration, or AGA stage.

Ultrasound in hair transplantation

Objectives: The use of ultrasound in the diagnosis and treatment of scalp conditions and hair transplantation is indeed an exciting advancement in the field of aesthetic medicine and hair restoration. These advancements likely involve improved imaging techniques, such as ultrasound, to provide more accurate and non-invasive diagnostic information. Our team is exploring the use of ultrasound specifically on the scalp for diagnosing and preventing complications during hair transplantation.

Introduction: In recent years there has been a clear advance in the diagnosis of diseases of the scalp and in the field of hair transplantation. Information about the use of ultrasound in this area is scarce and therefore research in the area of imaging is necessary. This application of ultrasound can provide valuable insights into the condition of the scalp, allowing for more precise and safer hair transplantation procedures. Our work aims to demonstrate that is necessary during procedure to have a safety margin during surgery.

Materials / method: In ultrasound, the epidermis, dermis and subcutaneous layers reveal different densities between patients, with the epidermis in the frontal region being narrower than the occipital region and the distance from the epidermis to the periosteum approximately 4mm. The epidermal and dermal line is hyperechoic and the subcutaneous hypoechoic. In the background we have a line that corresponds to the bone margin and through echodoppler we are able to create a vascular map.

Results: We show that the distance between the epidermis and the bone margin corresponds to 4.5 mm without tumescence, and the punch used corresponds to 4 mm in length. After the introduction of tumescence solution, the distance between the epidermis and the bone margin corresponds to 7.0 mm, that is, a safety margin of approximately 2.5 mm was created. Don't having a safety margin contributes to possible aggression both at the periosteum and subcutaneous level. Also we know that subcutaneously we have a vast vascular network and injections based on lidocaine and adrenaline can be dangerous.

Conclusion: Dermatological doppler ultrasound is a non-invasive technique that allows the evaluation of scalp pathologies and the prevention of complications during hair transplantation. This diagnostic tool makes it possible to increase knowledge in the trichology area and, in the future, reduce the need for invasive tests such as scalp biopsy. In hair transplantation, it can be used during surgery, adding more safety to the procedure, optimizing the surgery and carrying out vascular mapping, also avoid intravascular injections and thus prevent future complications or intraoperative complications.

Efficacy of microneedle RF in the treatment of AGA: clinical evaluation

Objectives: 1. Classification of patient Hair loss
2. Treatment of the patient with a novel microneedle radiofrequency device for hair loss
3. evaluate the safety and efficacy of the treatment in inducing hair growth and preventing hair loss.

Introduction: Androgenetic alopecia is the most common form of alopecia affecting both males and females. In the course of life can affect up to 50% of women and 80% of men causing a progressive hair shedding, hair thinning and reduction of hair pigmentation. Numerous treatments, with variable results, have been proposed to promote hair growth and prevent hair loss.

Materials / method: Several patients received 5 microneedle radiofrequency treatments every 3 weeks and were followed up 6 weeks after the last treatment. All patients were evaluated by global photography.

Results: Patients demonstrated faster hair growth, fuller hair, an increase in hair thickness and a reduction in hair shedding. The treatments were well tolerated by all patients. No adverse events were observed.

Conclusion: Microneedle radiofrequency is effective and safe for hair growth stimulation in AGA. Further research is needed to optimize the treatment protocol and evaluate possible combination therapies.
