Objectives: Demonstration of the systematization of vulvar filling with hyaluronic acid through which the filling areas were mapped to deliver better results.
Introduction: Labia majora aging leads to volume loss, sagging, and aesthetic discomfort. Hyaluronic acid (HA) fillers offer immediate volumization, improving local contour, protection, and patient confidence. However, incorrect indications or excessive volume can lead to undesired outcomes, including unnatural appearance and discomfort.
Materials / method: HA gel was injected in the labia majora using a 22G, 5 cm cannula. Patient selection prioritized those with volume loss but minimal flaccidity. For flaccid tissue, prior treatment with collagen biostimulators was recommended. Total volume varied between 1.5–3.0 mL per side.
Results: Patients experienced immediate improvement in labial contour and support, with high satisfaction rates and low complication incidence. In properly selected cases, HA provided natural-looking results and restored volume without bulking or mimicry of testicular appearance. Outcomes were enhanced when combined with biostimulation.
Conclusion: Hyaluronic acid is a valuable tool for labia majora rejuvenation when used judiciously. Proper patient selection, volumization limits, and anatomical precision are essential to avoid complications and ensure aesthetic harmony. In cases of flaccidity, biostimulation should precede filling for optimal results.
