Lipbeauty art: a pattern to build an elegant lip shape

Objectives: Understanding a visual algorithm for lip filler treatment. Learning Step by step all the passages and techniques for creating natural looking lips with HA fillers

Introduction: Lip Augmentation with injectables is nowdays one of the world top three procedures in aesthetic medicine. Forced lockdowns with increasing time on social media have helped to spread different lip injection trends, many of them with unnatural looks. The Aesthetic Field must bring the lip injection treatment back to the path of true beauty.

Materials / method: Between September 2019 and October 2020, 100 female patients underwent lip filler augmentation using this shape construction for lip augmentation. Each subject underwent photography and completed 2 surveys (follow up at 6 months and 1 year) measuring self-perception of mood before treatment and after achieving an optimal cosmetic result. Aesthetic improvement with treatment was assessed using a 5-point Investigator's Global Assessment (IGA).

Results: Response to treatment after 6 and 12 months were observed in 93% and 90% of patients, respectively. The mean value of IGA score in month 6 and 12 were 4.6 ± 0.50 and 4.7 ± 0.78, respectively. The study subjects were all satisfied with their lip improvement lip shape is correct in frontal and lateral view in each case. Reported adverse effects were temporary and mostly mild in severity.

Conclusion: By using a visual algorithm to construct the best version of each individual lip shape, operators, despite of his/her experience and individual patient requests, could create correct shape with no bumps, duck shadow or incorrect proportion.

Ultrasound evaluation of inaesthetic overcorrection and deformities of upper lip after HA injections

Objectives: The purpose of this study is to describe the ultrasound (US) patterns of upper lip tissue changes induced by HA injections, which gave inaesthetic result: persistent edema over vermilion border, noninflammatory lumps of dry mucosa, wet mucosa sagging deformity.

Introduction: Due to the popularization of lip augmentation procedures, many lips overcorrection and noninflammatory deformities associated with the presence of hyaluronic acid (HA) are observed in practice. Previously, the lips anatomy has been studied with histology and sonography methods after hyaluronic acid injections. However, it is difficult to find an US study on lips deformities and inaesthetic outcomes induced HA injections.

Materials / method: This study presents US images analysis of five patients with various deformities of upper lip caused by HA, injected more than 3 months ago. The assessed changes were compared to US images after filler removal with hyaluronidase in four cases. Ultra-High Frequency 20MHz with linear probe was used to evaluate the deformities.

Results: The study revealed the heterogenous echogenicity and blurry boundaries of orbicularis oris muscle (OOM) with a cottony patterns of surrounding tissue. Based on the comparison US measurements of upper lip thickness after HA dissolving with hyaluronidase it was described the effect of tissue compression. The lumps was identified as local thickening of submucosal layer with increased echogenicity combined with dry mucosa bulge. The wet mucosa sagging deformity was a thickening of the submucosal layer in dry/wet mucosa junction, which didn’t pull up with OOM during the smile and hid the teeth.

Conclusion: The present study helps for physicians to understand the filler behavior in the lip tissue and identify different ultrasound patterns of the overcorrected upper lips and tissue deformities caused by HA filler for improving lips repairing and augmentation procedures.

Penile dermal filler augmentation

Objectives: The purpose of this presentation is to improve understanding of penis filler augmentation, a growing and worldwide trend. What are the indications? What is involved in the consultation? How is performed? What are the potential adverse outcomes and how are they treated? What are the results?

Introduction: The world of penis augmentation using dermal filler is an exciting new way to gain more male patients into your clinic, but also to explore more ways to improve self esteem. Almost every patient has reasons why they are interested in augmenting the size of their penis, and many men who are above average size visit to request augmentation. It’s important to be aware of the risks involved, understand the anatomy and feel confident in management of these prior to undergoing down the penis filler road in your clinical practice.

Materials / method: A powerpoint presentation will explain the above points of discussion. Ideally it would take approximately half an hour.

Results: it is an explanatory process- not a study.

Conclusion: Penis filler is a rapidly growing area in the world of injectables and has usurped the throne of surgical options for phalloplasty.

Serial fat grafting of face

Objectives: We aim to assess the outcome of cosmetic facial fat transfer for the purpose of volume restoration.

Introduction: Historically, use of autologous fat for the correction of facial depressed scars was proposed in 1893 by Neuber. In 1930s, fat transplantation fell out of favor due to unpredictable resorption rate, cyst formation and fibrosis. However, newer technical modifications have improved fat viability, making lipofilling widely popular.

Adipose tissue was considered an inert organ, until its endocrine and metabolic functions were unraveled. e. Injecting fat in areas of atrophy gives volumetric rejuvenation and its survival depends on technique and recipient site characteristics.

Materials / method: A prospective, interventional study involving 10 patients on facial serial fat grafting for up to 3 sessions was conducted in Nepal. demographic profile, clinical assessment in terms of photographic evidences and the Global Aesthetic Improvement Scale (GAIS) score of the enrolled patients were kept. 3 Sessions were repeated at 3 months interval in 5 patients, while 5 patients had only one session. Records of Photographic evidence & GAIS score per operatively, immediate post-operatively, at 3 months & one year were kept.

Results: Among total patients, 7 patients (70%) reported “exceptional” improvement (GAIS Score 1) immediately following fat grafting, while 9 (90%) had moderate improvement (Score 3).
In next one year, only 3 patients (30%) had Score 1, all of whom had 3 sessions done. While 2 (20%) had Score 4 and 5 (50%) had Score 3 after one year. Serial fat grafting had better volume restoration & patient satisfaction.

Conclusion: Serial fat grafting is promising new tool in anti-aging and cell based regenerative therapy for facial volume loss.

Safety and patient satisfaction outcomes using a novel HA filler for non-surgical rhinoplasty: a retrospective review of 984 treatments

Objectives: To evaluate patient satisfaction and adverse events in a large scale series of 984 consecutive nonsurgical rhinoplasty treatments, performed by a single injector, using a novel high G-prime injectable hyaluronic acid dermal filler (VYC-25L).

Introduction: Injectable dermal fillers are widely used for a variety of facial aesthetic procedures, yet their use in non-surgical rhinoplasty remains controversial. Selecting the product best suited to the procedure, technical expertise, and the potential for serious complications continue to be challenges clinicians face in optimizing outcomes associated with this high-risk procedure.

Materials / method: This was a single centre retrospective chart review of patients who underwent nonsurgical rhinoplasty between January 2020 and July 2022. Four anatomical areas on the nose were injected with filler using a combination of a 27G 13 mm needle and a 25G 38 mm cannula, for improvement in nasal projection, tip elevation and rotation, correction of dorsal hump, and/or adjustment to columella. Patient demographics, early and delayed adverse events and patient satisfaction using validated questionnaires (Face-Q nose and Face-Q outcome) were reviewed.

Results: 984 consecutive treatments were performed in 492 patients. Touch up treatment was required in all cases at 4-6 weeks. High satisfaction outcomes were recorded via Face-Q Nose (mean 90%) and Face-Q Outcome (mean 99.2%). Early transient adverse events included edema (100%) and bruising (25%). Residual asymmetry (3.6%) and localized vascular occlusion (0.6%) resolved with appropriate treatment. There were no cases of blindness, necrosis, infections, lumps, granulomas or delayed onset nodules during the mean follow up period of 11.1 months.

Conclusion: The author concluded that use of a proprietary high G-prime injectable hyaluronic acid dermal filler coupled with a systematic approach in non-surgical rhinoplasty technique, offered high patient satisfaction with low adverse events in this single centre large series.

Guidelines for botulinum neurotoxin injection for facial contouring

Objectives: he hypertrophied temporalis and masseter muscles give a muscular shaped and bulky contour to the face. Botulinum neurotoxin injection methods are commonly used for facial contouring; however, adverse effects have been reported owing to a lack of delicate anatomical information.

Introduction: The anatomical considerations when injecting botulinum neurotoxin into the temporalis and masseter muscles have been reviewed in the present study.

Materials / method: Current knowledge on the localization of the botulinum neurotoxin injection point with more recent anatomical dissection and modified Sihler's staining procedures was assessed.

Results: We found that for the muscles, the injection point can be more precisely demarcated. Optimal injection sites are presented for the temporalis and masseter muscles, and the injection technique has been suggested.

Conclusion: We propose the optimal injection sites in relation to external anatomical landmarks for the frequently injected muscles of the face to facilitate the efficiency of botulinum neurotoxin injections. In addition, these guidelines would aid in more precise practice without the adverse effects of botulinum neurotoxin.

Correction of gluteus volume with intramuscular filling

Objectives: To describe the methodology for correcting gluteal volume with long-term injectable implants performed in Brazil and already published in Plastic reconstructive surgery. An update with more than 900,000 cc of product infiltrated at the muscular level

Introduction: the search for the perfect butt has increased. Silicone prosthesis and liposculpture are not always possible. Minimally invasive procedures with fillers promote a natural result with minimal risk

Materials / method: Multicentric study carried out in Brazil (Sao Paulo, Rio de Janeiro and Porto Alegre). There were 16 years of follow-up and completion of filling at the intramuscular level aiming at the volumetry of the gluteus.

Results: More than 900,000 cc of long-lasting product (polymethylmethacrylate) were applied aiming at a volumetric gain at the intramuscular level with surprising results in volumetric gain where the product stays in the muscle supporting and not in the skin "weighing".

Conclusion: the results were considered very satisfactory and natural by the medical team and by the patients themselves, as we will demonstrate with the images and videos. Adverse effects were minimal and compared to established techniques such as liposuction and silicone prosthesis.
Filling seems to be the most natural, safe and promising technique for volumetric gain in the buttocks.

Introduction: THE SEARCH FOR AN IDEAL CORPORAL CONTOUR HAS SUFFERED A SIGNIFICANT INCREASE IN RECENT YEARS. IT IS KNOWN THAT GLUTEO IS GREATLY RESPONSIBLE FOR GIVING MORE OUTLINE AND FEMININITY TO WOMEN.
SO, OVER THE YEARS, WE HAVE PERFECTED THE TECHNIQUE OF FILLING GLUTEO WITH POLYMETHYLMETACRYLATE (PMMA) WITH THE PURPOSE OF INCREASE IN GLUTEO, CORRECTION OF FLACKNESS AND LIPODYSTROPHY AND ALSO CORRECTION OF TROCHANTERICAL DEPRESSION.

Materials / method: OVER 2,000 WOMEN SUBMITTED TO AN INCREASED BUTTONS IN THE INTRAMUSCULAR PLATE WITH PMMA. SIDE EFFECTS WERE CALCULATED, PICTURES REVIEW BEFORE E AFTER TO PROVE THE EFFECTIVENESS OF RESULTS. MONITORING TIME OF 13 YEARS, WE WILL PRESENT AVERAGE OF THE NUMBER OF SESSIONS REQUIRED FOR THE DESIRED RESULTS AND THE AVERAGE OF THE QUANTITY OF PRODUCT USED.

Results: PRESENTING A LOW RATE OF COMPLICATIONS, A GREAT DIFERENCE IN VOLUME AND SHAPE. A HIGH SATISFACTION RATE.
WOMEN BEING FOLLOWED OVER 13 YEARS WITH OVER 2,000 WOMEN.

Conclusion: GLUTEAL AUGMENTATION WITH PMMA IS A SAFE PROCEDURE IN A LONG FOLLOW UP YEARS.

Is thread lift effective?

Objectives: Thread lifting is the only non surgical but minimal invasive procedure for soft tissue repositioning. It is not a replacement for surgical face lift and patient selection is a key in maximizing the outcome.
The procedure has minimal down time and it most of the cases should be combined with other treatments such as neurotoxins or fillers
