Objectives: Perioral rejuvenation is limited by poor three-dimensional understanding of the labiomandibular fold (LMF). This study used micro-CT and histology on 22 specimens from 11 cadavers to analyze LMF anatomy. The LMF marks a boundary between fat-abundant and fat-scarce zones and is dynamically shaped by a slanted collagen-elastin-muscle (CEM) network involving depressor anguli oris, platysma, and depressor labii inferioris. Findings reveal that LMF formation involves subcutaneous tissue differences and vertical dermal muscle insertions, supporting region-specific rejuvenation strategies.
