Objectives: the indication, usage, effects, and complications of hyaluronidases in medicine, with a focus on its therapeutic uses in obstetrics

Introduction: Hyaluronidases are a group of enzymes that degrade hyaluronic acid, a basic component of the extracellular matrix. These enzymes have the ability to cause transient relaxation of connective tissue by depolymerizing and hydrolyzing hyaluronic acid, which is a glycosaminoglycan found in several tissues, including connective, epithelial, and neural tissues. Hyaluronidase has been widely employed in several branches of medicine, including esthetic medicine, reproductive medicine, oncology, obstetrics, and anesthetics.

Materials / method: The efficacy and safety of a perineal injection of hyaluronidase is explored through a review of different studies. Zhou et al. studied four randomized controlled trials. They found a lower incidence of perineal injury in women who received a perineal injection of hyaluronidase during the second stage of labor than in the control group. Hyaluronidase appeared to be safe and a potential for decreasing perineal tears.

In a previous report, primiparous women in labor were randomized to receive an injection of hyaluronidase in the posterior perineum or no treatment during the second stage

Conclusion: While hyaluronidases are safe, clinicians should consider the effectiveness of the drug against less invasive methods. Further studies need to be made to introduce the use of hyaluronidases in medical common practice.

Objectives: HIFU in esthehtic medicine

Introduction: HIFU in aesthetic medicine hes been known since 2007 when FDA approved Ulthera in noninvasive bleharoplasty .
In this presentation I will show the application of HIFU in women with Female sexual disorder (FSD) as well as the the external use of HIFU together with autologous fat injection (AFI) at the same time

Materials / method: 32 women with FSD

12 women HIFU & AFI

Results: The medium initial FSD assessed by Female sexual function index changed in the end assessment after 6 weeks from 20,6 to 31,4

Conclusion: Results are more than encouraging but to drawn reliable conclusion more research are needed

Objectives: Microfat, Nanofat grafting in genital rejuvenationObjectives :
Our goal is to show that microfat, nanofat grafting can be effective in this indication. This study aims to present our technique, analyzing effectiveness, patient satisfaction, and complications.

Introduction: Genital restoration is growing in recent years. It can improve vaginal dryness, mucosa trophicity, genito-urinary symptoms of menaupose (GSM) and lost of elasticity and volume of external genitalia.

Materials / method: Patients presenting vaginal trophic disorders, atrophy of labia majora, and GSM were included.
After abdominal fat harvesting (or internal face of the knees, or thighs) and fat graft preparation, a microfat and nanofat grafting was performed in the genital area.
8 ml of microfat was injected in the labia majora and 6 ml of nanofat in the vestibul and the first 3 centimeters of the vagina wall (posterior and lateral)
pH, Fridmann score and Female Sexual Distress Scale (FSD) were used to evaluate the results, the effectiveness and tolerance of the treatment.

Results: 50 patients benefited from this technique. A significative improvment of the Fridmann score, pH, and FSD scale was found for all patients, with particular benefit on druness, pH and dyspareunia.
No complication were found in our study.

Conclusion: This study demonstrate that functional disorders of intimate sphere can be treated by micofat, nanofat grafting.
This autologous procedure is able to provide a very good rejuvenation of genital area. This method was effective in all patients, and required a single session.
