Objectives: Discuss different options for managing tubular breasts anomaly with particular emphasis on managing it by lipografting.

Introduction: Tubular breast is not an uncommon anomaly that causes a lot of aesthetic discomfort for many female patients. Most procedures described are invasive and will cause disfiguring scars that annoys the patient. The newly described lipografting technique solves the problem in a minimally invasive way with very accepted results and a high satisfaction rate both for the patient and the surgeon.

Materials / method: A video will show how to inject fat and how to treat the constricting fibrous ring and correct the infra-mammary fold position by riggotomies. The operation is safe, simple , minimally invasive and can be done under local anaesthesia.

Results: Results will be shown an many patients, revision lipografting is done in about fifty percent of the patients.

Conclusion: Lipografting with riggotomies is a simple and safe way to treat tubular breasts that can be done under local anaesthesia with high patient satisfaction.

Objectives: To demonstrate the alternatives that plastic surgeons will have to resort to in the case
patients will request the removal of their silicone implants. plastic surgeons should master these techniques very well, know the indications , limitations and possible complications.

Introduction: In the early 1993, we faced a situation of females requesting the removal of their silicone implants following a panic correlating silicone to cancer and this was experienced for 5 years till the FDA study was completed and proved no relation of
silicone to the induction of cancer breast. As the recent episode of ALCL is reported
increasingly in different studies, we should be ready to the worse.
If media will create a public panic , we will face a real need to help these patients. Alternative will fox on the 3D rebuilding of the breast with microfat grafting.

Materials / method: The technique of fat harvest, manipulation and injection will be demonstrated. All the
details and possible complications will be illustrated.

Conclusion: fat grafting with the 3D building of the breast is a good alternative to replace the silicone if it will be mandatory to remove the implants in the future.
