Objectives: For the first time, a special session for young dermatologists will be held during the 21st IMCAS Annual World Congress.

Introduction: The program of this session has been specially developed following the favorite topics chosen by the young dermatologists.

Materials / method: This session will be divided into five parts, starting with melasma, its diagnosis and an update on available treatments in 2019. Then, we will move to the management of scars, and the role of early intervention by pulsed dye and CO2 ablative fractional lasers to improve them. Theoretical and practical lectures on fractional lasers and cryolipolysis will be teached by two main developers of these devices: Dr Dieter Manstein and Dr Hans Laubach. The fourth part will alert us on safety laser, and on the benefits of an early laser training for young practitioners.

Results: Finally, young dermatologists will have the opportunity to present their communication before a well-known jury. Several subjects will be addressed: mycobacterial infection after a facelift, the use of hyaluronic acid in premature ejaculation, the role of CO2 ablative laser on rhinophyma and familial Brooke-Spiegler syndrome, and also surgical cases with reconstructions of the upper lip, the medial canthus area, and the lower limbs with specific flaps. The jury will select one young dermatologist to receive the award of the best communication.

Conclusion: Besides the high-level scientific content, this session also aims to meet young dermatologists coming from the whole world, to create connections between countries, as well as to give the opportunity for them to present their work in an international congress.

Objectives: Presentation of a case of Disfiguring rhinophyma successfully treated by carbon dioxide laser

Introduction: Rhinophyma is the most common presentation of phymatous rosacea. It reflects hypertrophy of the sebaceous glands of the nose. The condition can cause large nodular growth, resulting in distortion of the nasal contour and cosmetic disfigurement. Treatment of Rhinophyma is usually challenging. Medical treatments include antibiotics and retinoids. While, surgical modalities include excision, electrocautery, radiofrequency, dermabrasion, and lasers.

Materials / method: A 69-year-old male patient presented with long-standing large disfiguring rhinophyma. The condition inflected a significant cosmetic problem and social distress for him. Treatment consisted of 4 sessions of carbon dioxide laser, spaced 2- 4 weeks apart. The procedure was performed under local anesthesia. Carbon dioxide laser was used for debulking of the nodular growth part by part in each session. Both continuous and scanner modes were used. This was followed by 2 sessions of fractional carbon dioxide laser spaced at 4 weeks, to improve skin texture and achieve good cosmetic appearance.

Results: After treatment, there was an excellent response, with marked improvement in erythema, bulk and texture of the nose. In addition, restoration of the normal nasal contour was achieved. The patient reported excellent satisfaction with the results. Side effects were minimal in the form of bleeding during the procedure which was stopped by compression, in addition to, erythema that resolved within 2 - 4 weeks. Healing and reepithelization occurred within 2 - 4 weeks.

Conclusion: Ablative carbon dioxide laser can be effective in management of severe cases of disfiguring rhinophyma. It can debulk and sculpt the nose, with minimal complications and high patient’s satisfaction. Ablative carbon dioxide laser can be used for initial debulking of rhinophyma. This can be followed by fractional carbon dioxide laser resurfacing sessions to improve the texture of the skin and reach satisfactory cosmetic outcome.
