Advanced fillers: the use of polymethylmethacrylate for facial and body volumization – an 18 year retrospective

Objectives: By the end of this session the participant will be able to:
• Appreciate the artistic aspect of facial and body contouring and/or volume
replacement using a permanent dermal implant.
• Understand and select the ideal candidate for a permanent dermal
implant.
• Understand the pearls and pitfalls of injection technique.
• Understand that the longevity of PMMA can exceed 5 years.

Introduction: The demand for facial fillers has increased dramatically over the last 20 years. This is partially due to the less invasive, less downtime nature of these procedures. Other than the long established use of hyaluronic based fillers, there are biocompatible “permanent” fillers used in the field of aesthetic dermatology. One of the most widely used is PMMA. Although FDA approved for wrinkles and folds on the face, with proper technique, it can be considered as a long-term, viable alternative for patients seeking to correct volume loss and contour irregularities on the body.

Materials / method: A retrospective chart review was conducted of 22 patients who received PMMA for facial and/or body volumization between 2003 and 2021. The patients were evaluated both by serial photographs, global aesthetic patient satisfaction scores and whether or not they would either recommend the procedure or repeat the procedure in the future.

Results: Of the 22 patients treated, 1 was unsatisfied although there was notable improvement as evidenced by photographs. One patient developed a granuloma that had to be excised but the patient satisfaction score was still high. 2 patients had other treatments (radiofrequency, neuromodulators, threads) in conjunction with the PMMA. In 91% of the patients, the PMMA lasted more than 5 years with only 2% requesting repeat (touch-up) treatments.

Conclusion: PMMA-collagen is a long-lasting dermal filler approved by the FDA to treat deep skin folds and wrinkles on the face. However, given proper injection technique, PMMA-collagen can be used off label for both facial and body contouring as a less invasive, alternative method of restoring lost volume and contour. As a permanent dermal implant, it has proven to have both a very good safety and satisfaction profile. Although PMMA is the only FDA approved filler with a 5 year durability, patients should be made aware that it can last (at least) up to 10 years in some cases.

The challenge of reposition tissues without volumizing

Objectives: Injectable fillers are usually considered to be used to fill wrinkles, folds or localize areas of volume loss. However, we can achieve lifting effects while using the in lateral aspects of the line of ligaments.
For this reason, we wonder if using filler injections in lateral area we can reposition facial structures with minimal quantity of filler. We want generate a new protocol based on the line of ligaments to improved results to minimized volumizing treatment.

Introduction: Age-related changes are observed in all tissues. Rejuvenate procedures should aim patient´s safety, deeply understanding of anatomic layers, and obtain long lasting results.
The imaginary vertical line that connects 4 major facial ligaments: temporal ligament of adhesion, lateral orbital thickening, zygomatic cutaneous ligament, and mandibular ligament. This line separates the face into a medial and lateral side.
We propose to infiltrate fillers into the lateral part for lifting effects, and into the medial part for volumizing effects.

Materials / method: We treated 8 patients. The aim was to treat laxity and sagginess in the lateral part and volume loss in medial midface. We infiltrated a high cohesive hyaluronic acid into subcutaneous and supraperiosteum level. The product was placed in 0.1-0.5ml boluses or 0.5ml fan technique.
We lift these major 4 ligaments with injections in these points:
- Temporal cavity: we injected in supraperiosteum with gunshot technique, 0.5 ml
- Zygomatic arch: we injected 3 boluses in supraperiosteum
- Pre-parotid area: we injected 0.5 ml
- Mandibular angle: we injected 0.3-0.5ml in supraperiosteum

Results: Four patients were injected following our ligament line model. In these patients, we can observe improvement in nasolabial and nasogenian folds, relocation of jowls and cheek projection. As we can show in images, the injection of hyaluronic acid in medial part to volumize were not necessary.
On the other hand, four patients were treated in lateral and medial areas. We can observe that when we firstly injected in lateral areas, we need less hyaluronic acid to volumize in medial areas. Only, it was necessary inject 0.3-0.5 ml of product in medial areas, minimizing “puffy” faces outcomes.

Conclusion: Our ligament line proposed model is based in anatomic points. This model shows that lateral injections result in the stretching and flattening effect of the midface soft tissues and fat compartments get smoothed by the applying stretch on the facial layers.
The main result of this study is to make real emphasize in the importance of facial anatomic boundaries, where soft tissue fillers can be placed not only safely without distorting anatomy or creating puffy faces, but also using less amounts of filler when the first target are lateral injection points.

Drug injections to reduce facial fat

Objectives: Demonstrate the effectiveness and safety of a cocktail of injectable botanical substances (caffeine, L-carnitine, green tea, cynarascolymus extract (artichoke), pineapple sativus extract and Camellia sinensis extract (green tea) in reducing facial fat.

Introduction: The desire to reduce facial fat or thin the face is frequent in people with a round face or in the eastern population. Several technological or surgical alternatives are used for this purpose. There was still no study on the use of injectable botanical chemicals to reduce facial fat.

Materials / method: 40 patients aged 20-65 years underwent four to six sessions of injectable botanical substances (caffeine, L-carnitine, green tea, cynarascolymus extract (artichoke), pineapple sativus extract and Camellia sinensis extract (green tea), with weekly intervals in areas of more prominent fat such as cheek, jaw, soof and submental.

Results: All patients had some degree of decrease in facial or submental fat when observed in photographs. There was an improvement in skin quality in all patients. 60-90% of patients reported a good to excellent satisfaction rate. There are no side effects, with the exception of bruises and burning on application.

Conclusion: After a year of follow-up and with a high level of safety and patient satisfaction, we believe that this therapy will be part of the treatment options for rejuvenating and reducing fat and facial contours.

Introduction: The periorbital area one of the most eye-catching aesthetic units, and one of the first unties to show the signs of aging. Brow position is very important as it can either highlight the beauty of the eye or send a negative impression of anger, sadness, tiredness, or surprise and low intelligence. through history we saw different preference in brow shape from the half circle very high arched brow to the now a days more low thick lateral lifted brow. so what are the beauty concepts we can depend on to decide on brow lifting. how can we use botulinum toxin & fillers to achieve good brow lift.

Materials / method: Review of literature on beauty concepts, periorbital aging factors, brow lifting techniques , and brow shape preferences.

Results: Setting main rules for a good brow shape.

Conclusion: Although there are many fluctuation through history, even through the past few years, yet there are certain rules that we can use to get a good brow lift for a more youthful and beautiful look.

Hyaluronic acid in female genital rejuvenation

Effectiveness of the high-dose pulsed hyaluronidase management protocol in the treatment of impending skin necrosis

Objectives: 1- Identify the signs and symptoms of impending skin necrosis.

4- Compare the effectiveness of different treatment modalities in managing vascular occlusions.
5- Learn how to utilize the high dose-pulsed hyaluronidase treatment protocol effectively.

Introduction: Hyaluronic acid filler procedures have been consistently surging in popularity over the past few years. Complications are on the rise too paralleled by the increased demand for aesthetic treatments. This review intends to summarize the current evidence regarding the prevention and management of impending skin necrosis resulting from inadvertent intravascular injection of hyaluronic acid fillers. The aim of this systematic review is to evaluate the effectiveness of the high-dose pulsed hyaluronidase treatment protocol in preventing skin necrosis and reversing tissue ischemia.

Materials / method: An online search of the computerized bibliographic databases PubMed, and Embase was carried out. Included articles should report impending skin necrosis after hyaluronic acid filler injection in the head or neck region.Studies discussing other vascular complications, the use of fat grafts or non-hyaluronic acid fillers were excluded. The PICO format was used to frame the research question, and the PRISMA guidelines were used to report the search results.The initial search identified 3039 total studies from both databases. A total of 72 studies and 186 cases were included in the final analysis.

Results: The most common outcome in immediate intervention cases (< 24 hours after injection) was full recovery (39%). Most early intervention cases (1-3 days after injection) progressed to necrosis and later recovered without permanent sequelae (49%). Late intervention cases (>3 days) were the most likely to result in necrosis and scarring (83%). Hyaluronidase was used in 113 cases (61 %) in various doses. Low doses of hyaluronidase yielded unpredictable results. The high dose pulsed hyaluronidase protocol was used in 7 cases. All the cases resolved without residual scarring.

Conclusion: Immediate and early intervention cases had better prognosis when compared to late intervention cases regardless of the type of treatment used. The high dose pulsed hyaluronidase management protocol seems to provide predictable and satisfactory outcomes in impending skin necrosis caused by hyaluronic acid fillers. Initiating the protocol within 24 hours after filler injection halts the progression of necrosis and prevents permanent sequelae. Further research on the effectiveness of the high-dose pulsed hyaluronidase protocol in cases with late presentation is recommended.
