Introduction

Evaluation of a new technique for anchoring lower eyelid subcutaneous to lateral orbital periosteum in improving blepharoplasty

Objectives: Although lower blepharoplasty is one of the most common requests from people in search of beauty, it is one of the most challenging techniques in the field of plastic surgery because it may be associated with subsequent complications so there is not pleasant for the surgeon. The objective of this abstract is to share the personal experience on a new technique.

Introduction: Although a detailed study has not yet been performed, it may the significant rate of ectropion after lower blepharoplasty be related to lack of eyelid support so according to some theories, lateral cantopexy concomitant with blepharoplasty, decrease the incidence of this condition. In addition, it seems that anchoring the subcutaneous tissue of the lower eyelid through a permanent suture to the periosteum outside the orbit can further increased the support of the lower eyelid, which is an innovative technique and the basis of this study.

Materials / method: The patients underwent lower blepharoplasty with percutaneous subciliary technique and then lateral cantopexy perform as fixation of the external retinaculum to the supralateral orbit periosteum. Subsequently, as a new technique, the subcutaneous tissue of the eyelid is anchored laterally to the lateral periosteum with non-absorbable suture. Follow-up visits are performed after one week, one month and six months later.

Results: In these sessions, quality structures, including patient satisfaction, eyelid symmetry, crease status, lateral commissure status, visual acuity and possible complications, especially ectropion is performed and finally the collected data will be analyzed.

Minimally invasive diced cartilage aesthetic correction of the botched nose

Objectives: 1) Know the least invasive technique for grafting in over-resected secondary rhinoplasty cases 2) acknowledge the use of the advanced technology in surgical theater to reduce morbidity and increase success rate

Introduction: Cartilages are the most required materials for grafting in secondary rhinoplasty cases with over resection in multiple structural and surface defects. The selection of the ideal graft has been an issue of concern for its donor site morbidity and other possible problems such as graft visibility and irregularities, infection, and inflammatory reactions.

Materials / method: Advances in facial plastic surgery persuaded us to use special techniques to decrease donor site morbidity using autografts. Combining open and endoscopic approaches can provide adequate support for structural and camouflage purposes exclusively from the non-rib sources with the most negligible morbidity and successful results. Diced cartilages from the remnants of the non-rib origins can be wrapped in fascia temporalis, avoiding any graft harvest from the rib and other distant areas. This presentation will focus on the minimally invasive technique of non-rib cartilage wrapped in the facia.

Results: Details of the procedure are described through illustrative pictures and videos of the operating view, emphasizing the pearls and pitfalls in every stage. Here are some advantages of this selection: (1) It avoids an incision in the chest to harvest rib cartilage. Using minimal cartilage from diﬀerent sources, we can gather a large enough volume to be wrapped in the fascia. (2) It is less visible over time due to its soft and round edges. Visibility of graft edges is more common with solid grafts.

Conclusion: (3) There is decreased risk of torsion and warping of the graft, which is of particular concern with rib cartilage grafts. (4) The surgeon can completely attach the graft to the recipient bed, ensuring more stability. (5) These grafts are compatible with an asymmetric bed with bony and cartilaginous irregularities and eﬀectively camouﬂage such asymmetries. (6) Even in postoperative visits, the soft cartilage fascia graft is ﬂexible and compliant enough to be massaged and modified meticulously.

Baker-IV capsular contracture is correlated with an increased amount of silicone material: an intra-patient study

Objectives: Learn the origin of Baker 4 capsular contraction around a breast implant
Learn how to prevent Baker 4 capsular contraction

Introduction: Breast implant surgery is one of the most frequently performed procedures by plastic surgeons worldwide. However, the relationship between silicone leakage and the most common complication, capsular contracture, is far from understood. This study aimed to compare Baker-I with Baker-IV capsules regarding their silicone content in an intra-donor setting, using two previously validated imaging techniques.

Materials / method: Twenty-two donor-matched capsules from eleven patients experiencing unilateral complaints were included after bilateral explantation surgery. All capsules were examined using both Stimulated Raman Scattering (SRS) imaging and staining with Modified Oil Red O (MORO). Evaluation was done visually for qualitative and semi-quantitative assessment and automated for quantitative analysis.

Results: Using both SRS and MORO techniques, silicone was found in more Baker-IV capsules (8/11 and 11/11, respectively) than in Baker-I capsules (3/11 and 5/11, respectively). Baker-IV capsules also showed significantly more silicone content compared to the Baker-I capsules. This was true for semi-quantitative assessment for both SRS and MORO techniques (p=0.019 and p=0.006, respectively), while quantitative analysis proved to be significant for MORO alone (p=0.026 vs. p=0.248 for SRS).

Conclusion: In this study, a significant correlation between capsule silicone content and capsular contracture is shown. An extensive and continued foreign body response to silicone particles is likely to be responsible. Considering the wide-spread use of silicone breast implants, these results affect many women worldwide and warrant a more focused research effort.

More than 10 years experience on hi-def liposculpture: 10 golden rules to avoid complications and unsatisfactory results

Objectives: Although plastic and aesthetic surgery is a very comprehensive branch, many physicians are increasingly focusing on special areas and developing themselves. Advanced body shaping is also one of these areas. Both with the help of developing technology and the pressure of patient demand in this direction, surgeons' interest in this field is extremely intense. This intense interest and rapid progress in technology have revealed a long learning curve in order to be successful in this field.

Introduction: As a surgeon who has been dealing with body surgery for more than 10 years, here are the 10 basic rules I have set step by step in order to keep patient satisfaction at the highest level and to stay away from complications. While determining the 10 basic rules, 119 patients (67 female, 52 male) among the patients who have undergone surgery in the last 7 years, according to their dermographic characteristics, were evaluated with BMI, height, kg, vaser working time, forehead fat ratios, Hgb-htc value changes, MRI examinations and satisfaction factors ,evaluated and recorded.

Materials / method: 1 .Patient Eligibility : The first to consider when making patient selection. The point is patient expectation. Patients whose expectations cannot be met scientifically are not suitable for this operation. Other important points can be listed as subcutaneous fat tissue localization, skin quality, muscle and bone structure. It should be kept in mind that with the right evaluation, the right patient will meet with the right procedure.

Conclusion: As a surgeon our main concern is to acquire best results without complications. Hi-def liposculpturing is an agressive surgical approach therefore from all arround the world we have hearing comlications such as burns , hematoma , infections , even more worsen . In order to avoid complications , during last decade ı created my own philosophy which based 10 simple steps. Regardless some technology this steps are necesarryly required.

Crecentric inferior areolar de-epithilialized dermal support to minimize nipple retraction in gynecomastia

Introduction: The perfect body shape, has been recently increased in the last few years and became one of the major concerns for both young adults, male and female probably due to increased merchandize of the optimum body image posed by the social media

Combining high definition liposuction and abdominoplasty for a more aesthetic look

Introduction: The use of high definition liposuction technique has been on the rise worldwide. While a traditional lipo-abdominoplasty consist of removing skin, performing a diastasis repair and liposuction, high definition can play a key part in achieving an athletic frame that can enhance the transformation

Materials / method: In this presentation I will present what I have learned from 200 cases of combined abdominoplasty and high definition liposuction

Results: I will present my patient selection., drawings, surgical technique as well as the results.

Conclusion: By avoiding danger zones high definition liposuction can be combined sawfly with an abdominoplasty in order to achieve the desired athletic frame.

Autologous fat grafting: risk factors negatively influencing the take rate - A clinical study

Objectives: Determination of possible negative factors influencing the take rate in order to contribute to an optimization of autologous fat grafting as a valuable method in reconstructive breast surgery in the future.

Introduction: In reconstructive breast surgery, autologous fat grafting is considered an excellent minimal invasive and oncologically save method for the breasts’ volume reconstruction with autologous tissue. However, the grafted lipoaspirates are partially degraded by the body's own resorption processes, leading to a final take rate of only about 40%. Several sessions are therefore usually required to achieve satisfactory results. To date, no gold standard could be defined from numerous aspiration, processing, as well as infiltration techniques of autologous adipose tissue.

Materials / method: 10 patients undergoing breast reconstruction with autologous fat grafting after subcutaneous mastectomy were included. Punch biopsies and lipoaspirates were harvested and non-invasive lipometer measurements were performed to determine the take rate. Electron microscopy, histological and laboratory chemical analyses were performed to identify adipocyte size, structure and viability. Furthermore, various other patient-related characteristics were correlated with the take rate. Further 45 lipoaspirates obtained via various liposuction techniques were analysed regarding their quality and vitality.

Results: Various patient- and surgery-related risk factors were found to negatively influence the take rate in autologous fat grafting. Additional analysis of 45 lipoaspirates yields interesting results regarding the influence of different liposuction and processing techniques and the use of tumescent fluid on the quality and vitality of adipocytes. Qualitative differences of lipoaspirates can especially be visualized within images obtained via electron microscopy.

Conclusion: Knowledge of possible negative factors influencing the take rate is of utmost importance to achieve optimal results and reduce complications. Further clinical projects on our findings are essential, to contribute to a significant reduction of fat grafting sessions via an increase of the take rate.

Structural high definition liposculpture of neck and jawline

Objectives: learning clincal judgemnt about comsidering liposclapture for chin and jaw line region , indication of recruiting technologies to optimize and enhance result

Introduction: chin and jaw line lipodystrophies are one of most highly demand aesthetic procedure encountred , one of the leading factor in sucessful outcome is patient selection and then chose the right technologies which recriuted to enhance result and to optimize outcome.

Results: operative time found to be average of 1hour 30 minute treatment of chin ,Neck and Jaw line, recovery time 2weeks , complications stated as swelling , echymosis , fibrosis ,asymetry and marginal mandibular nerve transit injury , and overall satisfaction are examined and stated

Conclusion: structural liposcalpture of Neck and chin elected to the right candidate which benefit from liposcalpture neck lift using comprehensive technologies such as invasive Radiofrequency and ultrasound and explaining the technical guidlines to deliver sfae procedue with enhanced optimal outcome

Personal technique for nasal tip projection and support: caudal septal advancement

Objectives: A new technique to provide tip projection and support in rhinoplasty.
1. Determine the loss of tip projection, rotation and support
2. Evaluate the septal cartilage
3. Determine the amount of septal cartilage needed
4. Surgically advance the cartilage as described in the technique
5. Fix the alar cartilage structures to caudal septal advancement cartilage.

Introduction: Rhinoplasty is one of the most challenging operations in the era of plastic surgery. The most demanding part of the operation is the tip. Successful rhinoplasty depends on well balanced nasal tip support and projection. Many different techniques to solve the problem of tip support and projection have been proposed in years and some have been in routine use although controversies exist in the Plastic Surgery literature about their late results. We performed caudal septal advancement technique for patients who require a mild to significant increase in tip projection and tip support.

Materials / method: We’ve operated on 245 patients with this technique between 2002 and 2021. All patients were operated with open technique. In caudal septal advancement technique we first determine where the nasal tip must be positioned. We prepare an angled rectangular cartilage extending down to the caudal edge of septum, which would carry the nasal tip to desired position. The graft is advanced as required and fixed to the septum with 4 to 6 sutures. The stability of the graft at this stage is good. Then prepared alar cartilages are fixed to this graft in two points in desired projection and rotation.

Results: None of the patients lost the projection in late postoperative period and all were satisfied with the result.

Conclusion: With the described procedure, we present another alternative to managing the long-lasting tip projection and support without any major complication. The technique keeps the septal integrity and only the required amount of septum is used. The caudal septal advancement is an easy technique to learn and execute.

Silicone implants and mandibular erosion in patients with face contouring surgery

Objectives: - Common classic materials used for face contouring surgery
- Common problems of the classical materials that is used for mandibuloplasty and genioplasty for face contouring surgery

- Safe materials used for face contouring

Introduction: Mandibuloplasty and genioplasty for face contouring is widely accepted as safe procedure with little complications. The procedure can either be done by osteotomy or by using prosthesis. Silicone(polydimethylsiloxane) is the classical material used. However, complications like secondary infection, implant rupture and migration were commonly reported. Recently, a bioengineered polyether ether ketone (PEEK) applications based on patient’s scanner were introduced to avoid silicone complications. The aim of this study was to describe the complications of silicone implants in a series of case report

Materials / method: Patients with silicone implants following mandibuloplasty and genioplasty presented to our clinic with clinical signs and symptoms of complications. The patients were examined by CBCT scans to determine the relationship between the implants and the surrounding bony structures. After their permission pre-silicone implant CT scans were collected, studied, and compared with their CT scans post procedure.

Results: CBCT scans showed significant resorption of the mental region, and in one case, with damage to the root of the teeth, subsequently infection of the tissue has been observed. In another case a complete resorption of the cortical bone of the mandibular symphysis area was detected. In another case where mandibular angle implants were placed , lateral cortical bone resorption at the angle and ramus of the mandibule were observed.

Conclusion: Our results confirm the possible erosive side effect of implanted silicone in the mandible. We conclude that silicone implants are not suitable for mandibuloplasty. The implants were removed and the affected sites were reconstructed with osteotomies and PEEK implants.

Our experiences with lipocube nano for face and hand applications

Objectives: The aim of this study is to tell you with details the examples that prove that Lipocube Nano fat stem cell therapy is one of the most effective methods in face and hand regeneration and that it will be the most preferred treatment instead of other methods in the future with its permanence.

Introduction: Between 2018 - 2021 overall 42 patients underwent lipocube nanotransfer procedure as three layer approach. 18 of patients were hand and face only injections. 11 were blepharoplasty and nano fat injection together and 13 were with face lift and temple lift procedure.

Materials / method: Many of the patients were combination cases such as upper blepharoplasty or mini face lift together. All the face injections were three layer approach as milifat (2,4 mm<) for deep facial compartments to lift the ligaments and micro fat (1,2 mm<) for superficial fat replacements as chin, perioral area hands, forehead deep tear -trough and nano (400-600 micron) for the superficial subdermal and intradermal injections.

Results: Processes fat tissue in a closed sterile system with special blades enabling partial filtration of fibrous material. The processed fat contains fibers with regenerative cells, resulting in rapid revascularization of the fat in the injected area, thus very good graft uptake.

Conclusion: Here we would like to share our experiences by before and after pictures.
