Why clitoral hood plasty is important in the harmony of aesthetic female intimate surgery

Objectives: to show the impact of the clitoral hood plasty on the aesthetics and harmony in female genital area surgery.

Introduction: Cosmetic intimate surgery procedures are popular nowadays, patients are seeking to enhance their intimate anatomy to improve self-esteem, and confidence in preserving their genitalia function and sensations. A comprehensive approach to the female external genitalia by the combination of surgical techniques gives better results than isolated procedures.

Materials / method: The combination of non-surgical procedures such as hyaluronic acid volumetric injections to the labia majora and a combination of surgical techniques such as labia majora augmentation by autologous fat grafting, clitoral hood reduction plasty and labiaplasty when indicated were used to improve female genital aesthetic and functional outcome.

Results: For the last 9 years combined surgical and non-surgical procedures were performed. Aesthetic and functional outcome was evaluated with augmentation of self-esteem and genital function.

Conclusion: A comprehensive approach to the female external genitalia with the clever combination of available surgical and non-surgical techniques creates harmony, aesthetic, and functional results with high levels of satisfaction.

Vaginal injection of HA - The Vaginal Atrophy (VA) vectors, a new injection technique, simple and safer!

Objectives: Hyaluronic acid (HA) is a glycosaminoglycan widely distributed throughout the skin and mucous membranes, including vagina. Its topical use for vaginal atrophy is well established.

The injectable use of the HA, despite being widespread for facial aesthetics, is poorly documented in vagina. The few studies about vaginal injection (VI) however report encouraging results with improvement of atrophy, dryness and contact during sexual intercourse.

We developed a safer injection technique, that we used since 2019 and named Vaginal Atrophy (VA) vectors, with good results and no major complications.

Introduction: Biopsies after VI of HA have even shown an increase in the expression of two genes COL1A1 and COL3A1, demonstrating a fundamental role of the substance in trophism improvement of the mucosa.

However, the therapy has not spread due to 2 reported cases of pulmonary embolism associated with such application. It occured because vaginal wall, mainly anterior, is surrounded by extensive venous plexus. So to be safer, the injection has to be very superficial, in submucous plane. Besides that, it was injected overdosing of HA (5 - 15ml) and the procedures were performed by nonmedical practitioneres.

Materials / method: Since 2019, 22 women aged 35 to 70 have been injected with 1 ml of HA, 20mg/ml.

After passage of the speculum, asepsis and punctual anesthesia with 1% lidocaine, a 20G needle was punctured and made an entry port into the vaginal introitus, on the posterior wall, through which a 5cm 22G cannula was introduced superficially into the submucosa. HA was injected into 3 vectors, by retroinjection,1 central that received 0.4 ml and 2 lateral that received 0.3 ml each.

Results: All patients reported improvement in dryness and orgasmic potential. We had to dissolve the HA injected with hyaluronidase in 1 patient whose product accumulated very superficially, eroding after intercourse. We believe that we should have used a more viscous HA because the patient was very atrophic.

The injection was performed only in the initial 3 cm of the posterior wall of the vagina, since according to scientific studies and our daily practice, this is the place where pain and laxity bring the greatest discomfort, in addition to being the safest anatomic site for VI.

Conclusion: We believe that the Vaginal Atrophy Vectors Technique, or VA Technique, can be considered a safer way for the VI of HA, as it uses thick, blunt-tipped cannulas and recommends injection in safer anatomical points, as the superficial submucosa of the posterior wall. We strongly suggest prior aspiration, gently injections and never more than 1ml per session.

Regarding the choice of HA, when the complaint is vaginal laxity, a moldable HA should be injected, with greater volumizing power. In very atrophic patients, whose primary complaint is dryness, we suggest HA with greater hydration result.

Assessement of the efficacy of multifractional intercalated HA injection for genitourinary syndrome of menopause

Objectives: The aim of the study was to investigate the effect of multifractional intercalated hyaluronic acid (MIC-HA) injection on anterior and posterior vaginal wall thickness (VWT) using 3D-high frequency endovaginal ultrasound (3D-EVUS), and also to assess the relationship between sexual functions and VWT increase in women with genitourinary syndrome of menopause (GSM) after the injection.

Introduction: There are many studies reported short-term improving effects of topical HA on GSM symptoms and have used subjective assessment tools. However, there are no studies assessing the effect of injectable hyaluronic acid on the VWT ultrasonographically in women with GSM.

Materials / method: Thirty-one women with GSM were enrolled the study. After local anesthetic injection, 2 ml of MIC-HA was injected into the vaginal wall with a standardized injection technique. The vaginal health index (VHI), Turkish validated form of the Female Sexual Function Index (FSFI), anterior and posterior VWT measurements by 3D-EVUS were recorded before and 4 weeks after intervention.

Results: Four weeks after MIC-HA injection, VHI scores and total FSFI scores including all sub-scores were significantly higher than the baseline scores. Post-injection measurements of anterior and posterior VWT were found to be significantly increased compared with pre-injection VWT measurements (p=0.0001 and p=0.0001, respectively). While there was a positive correlation between increase rates of posterior VWT and satisfaction sub-score (r=0.436. p=0.014), desire and arousal sub-scores were positively correlated with increase rates of anterior VWT (r=0.357, p=0.038; r=0.389, p=0.031, respectively).

Conclusion: Measurement of anterior and posterior VWT with 3D-HFUS showed objective improvement in GSM symptoms after vaginal MIC-HA injection. The relationship between different components of sexual function and the increase in anterior and posterior VWT will provide a different perspective on the complexity of sexual function.

Vaginal and facial rejuvenation

Objectives: The pregnancy represent s big challenge for the women body and unfortunaly leaves permanent damage to body like stress incontinence. Until recently there was no succesful treatment for it. Many women are affected with that problem. Other big problem for the women is menopausa where many women due to the hormonal changes suffer from many problems like dryness, dyspaurenia and others. Thanks to the technology we can help to the women with no risk and improve their quality of the life.

Introduction: After pregnancy beside stress incontinence many women due the vagina damage lost the quality of the sex life. Vulvar region becomes hyperpigmented. Usully big and small labia become damaged and dilated. During menopausa there is atrophy of the mucosa and change of ph which promote vaginal rejuvenation.

Materials / method: The radio frequency represent the biggest game changer for the vaginal rejuvenation specialy after pregnancy and resolution of the postmenopausal problems. The latest generation of the devices use robot handpiece which rotates 360 degres and rejuvenate the vagina. With the other handpiece we treat vulvar area which represent nonsurgical labioplasty. The treatment is completly safe beacuse device use sensor to measure the skin temperature to avoid the burns. As final step we use use third handpiece to rejuvenate the face.

Results: It s recommend to have 3 treatments with the distance 3 to 4 weeks. The treatment is safe and with no down. Many women report significat results like improvent of the stress incontinence, remodeling of the labia, decrease of the hyperpigmentation, improvment of the dryness and the quality of the sexual life.

Conclusion: Vaginal rejuvenation with the radio frequency gives new hope and improvment of the life quality for many women.

Complex approach to women's intimate health by combination therapies

Objectives: The aim of this study is to provide an overview of existing non-invasive solutions that would be able to tackle these major vulvovaginal changes.

Introduction: The natural aggravating vulvovaginal changes are unfortunately a normal part of women’s aging described as vulvovaginal atrophy. It is characterized by a broad spectrum of genital (dryness, burning, and irritation), sexual (reduced lubrication, dyspareunia), and also urinary symptoms due to weak pelvic floor muscles (PFM).

Materials / method: To deliver a complex treatment solution for the patient, it is necessary to target both factors of women's aging, the weak pelvic muscles as well as the atrophying vaginal tissue. Weak pelvic muscles highly contribute to urinary symptoms and have a negative effect on sexual life. Vaginal atrophy symptoms negatively affect the intimate life of the patients by dyspareunia. A complex solution to intimate health appears to be the combo treatment with HIFEM technology that stimulates the pelvic floor muscles and the Monopolar RF that restores vaginal elasticity.

Results: Vulvovaginal changes appear to have a great impact on the QOL of affected women since these symptoms in most cases deteriorate if left untreated. The RF and HIFEM combo treatment leads to the improvement of vaginal laxity through collagen and elastin structural remodeling and the alleviation of UI symptoms due to PFM augmentation. Further, patients are highly satisfied with the mitigation of vaginal atrophy symptoms such as vaginal dryness, itching, and related dyspareunia, resulting in increased QOL including pleasure from their sexual and fitness life.

Conclusion: The combined treatment of monopolar RF and HIFEM procedure effectively enhances female pelvic floor health. The induced changes may have a serious impact on the subject’s quality of life by decreasing incontinence severity and improving sexual function.

Dermatological treatment using plant based makeup as a vehicle to repair, photoprotect, antioxidant and antiglycative for the skin

Objectives: The study aims to present an innovative and sustainable plant based vehicle for the skin in makeup form to treat and repair the skin against exposome effects, UVA, UVB radiation, visible light and blue light, DNA protection, antiglycative and collagen booster.

Introduction: Decorative cosmetic as makeup are used for application and coverage of skin defects, resulting in improved appearance as well as psychological effects such as increased self-confidence. Makeup usually contains heavy metals and endocrine disruptors that cause environmental pollution, skin irritation and contact dermatitis and normally can be not use for pregnant and sensitive skins. Pigments from plants in form of makeup are very innovative with strong antioxidant activities because contains flavonoids, tannins, polyphenols, anthocyanins that contains natural photoprotection.

Materials / method: It was developed a foundation in powder using natural pigments
B. excelsa seed powder, D. alata shell powder, O. sativa powder and R.sativus.It was evaluated the safety of the formulation using RIPT (Human Repeat Insult Patch Test) method, cytotoxicity and phototoxicity evaluation in vitro using cell cultures.To evaluate the efficacy of the formulation we used the follow methods:
DNA protection by ultra violet radiation using DNA damages detection.
SPF(Sun Protection Factor) in vivo using methodology ISO 24444.
UVA protective factor using Colipa method.
Collagen synthesis analysis.

Results: According to the results. The plant based foundation in powder is very safety and also can be considered hypoallergenic. By the SPF in vivo, the formulation can provide SPF 30 and UVA 21. The analysis of protection against blue and visible light showed that this foundation is very effective and the product present absorption of 74,1% from 400 nm until 800 nm (visible light) and 80,7% between 400 nm and 500 nm (blue light). The formulation stimulates collagen in 130,38% ± 0,1% in concentration of 1mg/ml.

Conclusion: In conclusion this makeup is a very innovative formulation to treat the skin with safety and efficacy. By the results this new technology can be used to improve the results of the dermatological procedures and protect the skin against exposomes, UVB, UVA, blue and visible light besides to improve collagen proliferation.
