Objectives: To present the rationale, indications, and clinical experience with the use of injectable biostimulatory agents—PLLA and PDLLA—for non-surgical buttocks augmentation, focusing on regenerative volume restoration and tissue remodeling.
Introduction: Non-surgical buttocks augmentation has become increasingly popular among patients seeking enhanced contour and firmness without the downtime and risks associated with surgery. Poly-L-lactic acid (PLLA) and poly-D,L-lactic acid (PDLLA) are biocompatible, biodegradable polymers that act as collagen stimulators. When injected in a diluted form into the subdermal or deep dermal layers, they provide gradual, natural-looking volume by stimulating neocollagenesis and improving skin texture. This approach offers a regenerative, progressive enhancement rather than immediate volumization.
Materials / method: Patients underwent buttocks augmentation using reconstituted PLLA or PDLLA suspensions injected via cannula into the subcutaneous tissue in a fanning or cross-hatch pattern. Injection plans were tailored based on anatomical assessment, volume requirements, and tissue quality. Treatment sessions were spaced 4–6 weeks apart, with an average of 2–3 sessions per patient. Post-treatment massage protocols were followed to ensure even distribution. Candidates were selected based on the absence of contraindications such as autoimmune disease, infection, or prior complications with fillers.
Results: Progressive volume enhancement and improved skin elasticity were observed over a 3–6 month period, with high levels of patient satisfaction regarding shape, firmness, and natural appearance. Results were subtle yet cumulative, aligning with patient expectations for a non-surgical solution. The treatment was well tolerated, with minimal adverse effects such as temporary nodularity or bruising. Maintenance treatments were considered after 18–24 months.
Conclusion: PLLA and PDLLA offer a safe and effective option for non-surgical buttocks augmentation, combining regenerative stimulation with gradual, natural contouring. These injectables represent a valuable tool in body aesthetic medicine, particularly for patients seeking subtle enhancement with minimal downtime. Proper injection technique, dilution protocol, and patient education are essential for achieving optimal outcomes.
