Introduction: Hair loss affects millions worldwide, significantly impacting self-esteem and quality of life. Conventional hair restoration techniques, including medications, PRP (Platelet-Rich Plasma), and hair transplantation, often yield variable results and may not be suitable for all patients. Recent advances in regenerative medicine have highlighted the potential of autologous fat grafting and adipose-derived cells (ADCs) in stimulating hair regeneration. Materials / method: Between 2018 and 2023, 68 patients (45 males, 23 females) with androgenetic alopecia or early-stage alopecia were treated with autologous fat grafting enriched with mechanically isolated adipose-derived cells. Fat was harvested from the abdomen or flanks using manual liposuction under tumescent anesthesia. The fat was washed and processed using a mechanical dissociation technique to isolate the stromal vascular fraction (SVF), containing ADCs, without the use of enzymes. Results: At 6 months, 81% of patients demonstrated increased hair density and improved hair shaft thickness in treated areas. Trichoscopic evaluation revealed an average hair count increase of 18-25% compared to baseline. Hair quality, scalp health, and patient-reported satisfaction showed notable improvements, with 76% of patients expressing satisfaction with the visible results at 6 months and 85% at 12 months.
