Objectives: Closed rhinoplasty has several advantages on opened way. This presentation describe this way, its limits, with cases and videos.
Introduction: We describe the indication of the closed rhinoplasty technique. Anatomic characteristics of the nose must be known to answer to an aesthetic rhinoplasty demand and to respect the functions.
Materials / method: Caucasian cases, but also Black African, Metis and Mediterranean noses are presented. Variations of the characteristics of nasal bone, cartilages, nostrils, and tip in several ethnics groups are also presented. We describe briefly nasal anatomy, a present patient complains and descript precisely the different therapeutic options chosen with the closed approach and the limits of it.
Results: Intercartilaginous incisions, alar cartilage resection, tip thinning, hump resection, lateral osteotomies, are, in my experience, generally performed under a closed way. Tip enhancement with cartilaginous graft, interdomal or interalar sutures can sometime done also by this way.
When cartilaginous graft of the upper lateral cartilage is required, when an important debulking of the tip is necessary, the open approach is preferred.
Conclusion: The understanding of the anatomic characteristics of the nose is necessary to answer to a patient complain and to respect functions, norms and beauty criteria, in a closed way. This way permit to treat majority of demand in my experience.
